补阳还五汤联合贝那普利对高血压合并脑梗死血流动力学、IL-6、TNF-α的影响

摘　　要：目的 探讨补阳还五汤联合贝那普利对高血压合并脑梗死的治疗作用。方法 将高血压合并脑梗死患者98例随机分组,对照组采用贝那普利;观察组采用贝那普利联合补阳还五汤。比较临床疗效及血流动力学、各项评价量表、生化指标,记录不良反应。结果 对照组有效率75.51%低于观察组91.84%(P<0.05)。与对照组比,观察组治疗后Vm、PI、RI较高,差异有统计学意义(P<0.05)。与对照组比,观察组治疗后收缩压、舒张压、NIHSS评分较低,MMSE量表较高(P<0.05)。与对照组比,观察组治疗后血ET-1较低,血NO较高,差异有统计学意义(P<0.05)。与对照组比,观察组治疗后血TNF-α、CRP、IL-6较低(P<0.05)。结论 补阳还五汤联合贝那普利对高血压合并脑梗死疗效确切,改善脑血流动力学,提高神经功能和血管内皮功能。Objective To explore the therapeutic effect of Buyang Huanwu Decoction(补阳还五汤)combined with benazepril on hypertension complicated with cerebral infarction.Methods A total of 98 patients with hypertension complicated with cerebral infarction were randomly divided into two groups.Benazepril combined with Buyang Huanwu Decoction was used in the observation group.The clinical efficacy,hemodynamics,various evaluation scales and biochemical indexes were compared,and the adverse reactions were recorded.Results The effective rate in the control group was 75.51%,lower than that(91.84%)in the observation group(P<0.05).Compared with those of the control group,the values of emean blood flow velocity of middle cerebral artery(Vm),pulsatility index(PI)and resistance index(RI)in the observation group were higher after treatment,and the difference was statistically significant(P<0.05).Compared with those of the control group,the levels of systolic blood pressure,diastolic blood pressure and the scores of National Institute of Health stroke scale(NIHSS)in the observation group were lower,and the scores of Mini-mental State Examination(MMSE)scale was higher(P<0.05).Compared with those of the control group,the serum endothelin-1(ET-1)in the observation group was lower and the blood nitric oxide(NO)was higher after treatment.The difference was statistically significant(P<0.05).Compared with those of the control group,the serum levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and interleukin-6(IL-6)in the observation group after treatment were lower(P<0.05).Conclusion Buyang Huanwu Decoction combined with benazepril is effective on treating hypertension complicated with cerebral infarction,improving cerebral hemodynamics,neural function and vascular endothelial function.
