风湿清合剂辅助治疗活动期类风湿关节炎对破骨细胞分化因子、趋化因子的变化研究

摘　　要：目的研究风湿清合剂辅助治疗活动期类风湿关节炎对患者血清破骨细胞分化因子(RNAKL)、趋化因子的变化影响。方法选取医院2019年1月—2021年2月收治的活动期类风湿关节炎患者90例,分为对照组(甲氨蝶呤)和观察组(风湿清合剂辅助甲氨蝶呤)。比较两组患者治疗后的临床效果。结果观察组ACR20和ACR50分别为91.11%(41/45)和57.78%(26/45),高于对照组的66.68%(30/45)和35.56%(16/45),但ACR70(33.33%,15/45)与对照组(20.00%,9/45)比较,差异无统计学意义(P>0.05)。两组治疗前RNAKL、趋化因子、RF、CRP、IL-17水平组间比较,差异无统计学意义(P>0.05)。观察组治疗后RNAKL、CX3趋化因子配体1(CX3CL1)、CXC趋化因子配体16(CXCL16)、类风湿因子(RF)、C反应蛋白(CRP)、白细胞介素-17(IL-17)水平低于对照组(P<0.05)。两组治疗前28个关节炎疾病活动度(DAS28)评分比较,无统计学意义(P>0.05)。观察组治疗后DAS28评分[(2.20±0.34)分]低于对照组(P<0.05)。结论风湿清合剂辅助治疗活动期类风湿关节炎可减轻炎症反应,调节RNAKL、趋化因子、RF的表达水平,降低疾病活动度。Objective To study the effect of Fengshiqing Mixture(风湿清合剂)adjuvant treatment of active rheumatoid arthri-tis on the changes of serum osteoclast differentiation factor(RNAKL)and chemokines.Methods A total of 90 patients with active rheumatoid arhritis admitted to the hospital from January 2019 to February 2021 were selected and divided into a control group(methotrexate)and an observation group(Fengshiqing Mixture assisted with methotrexate).The clinical effects of the two groups were compared after treatment.Results The ACR20 and ACR50 of the observation group were 91.11%(41/45)and 57.78%(26/45),respectively,which were higher than 66.68%(30/45)and 35.56%(16/45)of the control group,but ACR70(33.33%,15/45)was not stistically significant compared with that(20.00%,9/45)of the control group(P>0.05).The lev-els of RNAKL,chemokines,rheumatoid factor(RF),C-reactive protein(CRP)and interleukin 17(IL-17)before treatment in the two groups were not statistically significant(P>0.05).After treatment in the observation group,the levels of RNAKL,CX3 chemokine ligand 1(CX3CL1),CXC chemokine ligand 16(CXCL16),RF,CRP and IL-17 were lower than those of the control group(P<0.05).The comparison of 28 arthritis disease activity(DAS28)scores before treatment in the two groups was not sta-tstically significant(P>0.05).The DAS28 score[(2.20+0.34)points]of the observation group after treatment was lower than that of the control group(P<0.05).Conclusion Fengshiqing Mixture adjuvant treatment of active rheumatoid arthritis can reduce inflammation,regulate the expression levels of RNAKL,chemokines,and RF,and reduce the disease activity.
