摘 要:目的观察临床上已取得明显疗效的妇炎舒胶囊对盆腔炎性疾病(pelvic inflammatory disease,PID)模型大鼠子宫组织中细胞凋亡相关因子的含量及表达影响,探讨妇炎舒胶囊治疗PID的作用机制。方法选取42只8周龄SPF级雌性大鼠,采用混合菌液注射+机械损伤法建造PID大鼠模型,结合大鼠子宫炎症病理改变评分进行造模验证。给予妇炎舒胶囊高、中、低剂量、康妇炎胶囊、头孢克肟胶囊连续治疗14 d后,HE染色观察大鼠的子宫病理切片,运用聚合酶链反应(RT-PCR)检测子宫组织前B细胞克隆增强因子(pre-B-cell colony-enhancing factor,PBEF)mRNA的表达,酶联免疫吸附法(ELISA)检测子宫组织中B淋巴细胞瘤-xl(B cell lymphoma-xl,Bcl-xl)、半胱氨酸蛋白酶3(cysteinyl aspartate-specific protease 3,Caspase-3)、PBEF等因子的含量变化。结果(1)与空白组相比,模型组大鼠子宫炎症病理评分显著升高(P<0.05),子宫组织切片镜下见大量中性粒细胞浸润,子宫组织中Bcl-xl、PBEF浓度升高(P<0.01),Caspase-3浓度降低(P<0.05),提示造模成功,盆腔炎性疾病的发病机制可能与细胞凋亡相关;(2)与模型组相比,妇炎舒高剂量组子宫组织中Bcl-xl、PBEF浓度降低(P<0.05),Caspase-3浓度升高(P<0.05);(3)相关性分析:PBEF的表达与Caspase-3的表达呈负相关,相关性系数=-0.364(P<0.05)。结论PID模型大鼠的发病机制可能与细胞凋亡机制相关,妇炎舒胶囊可通过降低PBEF表达,同时下调Bcl-xL蛋白的表达水平,进而上调Caspase-3表达,调节炎性细胞凋亡,最终达到治疗PID的作用机制。Objective To observe the clinical ffeet of Fuyanshu Capsules(妇炎舒胶囊)on the contents and expressions of apoptosis-related factors in the uterine tissue of pelvic inflammatory disease(PID)model rats and explore the mechanism of Fuyanshu Capsules in the treatment of PID.Method Forty-ltwo8-week-old SPF female rats were selected,and the PID rat model was constructed by the mixed bacterial liquid injection+mechanical injury method,combined with the uterine inflammation and pathological changes scores for model verification.After 14 days of continuous treatment with high,medium and low doses of Fuyanshu Capsules,Kangfuyan Capsules and Cefixime Capsules,the pathological sections of the uterus of rats were observed by HE staining.The expression of pre-B-cell colony-enhancing factor(PBEF)mRNA in uterine tssues was detected by RT-PCR.ELISA method was used to detect the changes of B cell lymphoma-xI(Bel-xI),cysteinyl aspartate-speific protease 3(Caspase-3),PBEF and other factors in uterine tssue.Result①Compared with those of the blank group,the pathological score of uterine inflammation in the model group was significantly inereased(P<0.05).A large number of neutrophil infiltration was seen under uterine tssue biopsy,the concentrations of Bcl-xI and PBEF in uterine tssue were significantly increased(P<0.01)and the concentration of Caspase-3 decreased(P<0.05),indicating that the model was successful,and the pathogenesis of PID may be related to apoptosis.②Compared with those of the model group,the concentration of Bel-xl and PBEF in the uterine tis-sue of the Fuyanshu Capsules high dose group was significantly decreased(P<0.05),and the concentration of Caspase-3 sig-nificantly increased(P<0.05).③Correlation analysis:The expression of PBEF was negatively correlated with the expression of Caspase-3 and the correlation cofficient was-0.364(P<0.05).Conclusion The pathogenesis of PID model rats may be relat-ed to apoptosis-related pathways.Fuyanshu Capsules can reduce the expression of PBEF and down-regulate the exp
