龙砂膏滋方对无症状HIV感染者疗效及生活质量影响研究

摘　　要：目的探究龙砂膏滋方对无症状人类免疫缺陷病毒(HIV)感染者疗效及生活质量影响。方法将符合纳入标准的60例无症状HIV感染者随机分为观察组和对照组,每组30例,对照组经辨证接受抗病毒联合参灵扶正胶囊方案治疗,观察组于冬至日开始停服参灵扶正胶囊并改用龙砂膏滋方调治,服药时间约为30 d,服完膏方后继续给予抗病毒联合参灵扶正胶囊方案治疗,观察半年,比较各组患者临床治疗效果、治疗前后患者中医证候(发热、咳嗽、乏力、纳呆、腹泻、呕吐等)积分变化、T细胞亚群指标(CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+))变化、生存质量评分、不良反应情况、患者满意度,随访6个月,比较两组患者机会性感染发生率。结果较对照组患者(76.67%,23/30),观察组患者治疗总有效率(96.67%,29/30)更高(P<0.05);治疗后各组患者中医证候(发热、咳嗽、乏力、纳呆、腹泻、呕吐等)积分、生存质量评分、T细胞亚群指标(CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+))均改善,观察组患者治疗后中医证候(发热、咳嗽、乏力、纳呆、腹泻、呕吐等)积分、生存质量评分均优于对照组患者(P<0.05),但两组患者治疗后T细胞亚群指标(CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+))指标无差异(P>0.05);两组患者治疗过程中均未发生不良反应(P>0.05);观察组患者治疗满意度提升率(96.67%,29/30)高于对照组患者(76.67%,23/30)(P<0.05);观察组患者治疗后6个月内机会性感染发生率低于对照组患者(P<0.05)。结论龙砂膏滋方可提升HIV感染者临床治疗效果,改善患者症状,患者恢复较好,无不良反应,安全可靠,且患者生活质量、满意度显著提升,机会性感染发生率较低,值得推广应用。Objective To explore the effect and quality of life of asymptomatic human immunodeficiency virus(HIV)infected patients treated with Longsha Zhuzi Prescription(龙砂膏滋方).Methods Sixty asymptomatic HIV infected patients who met the inclusion criteria were randomly divided into observation group and control group,30 cases in each group.The control group re-ceived antiviral treatment combined Shenling Fuzheng Capsule(参灵扶正胶囊)according to syndrome differentiation.The obser-vation group stopped taking Shenling Fuzheng Capsule and changed to Longsha Zhuzi Prescription on the winter solstice for about 30 days.After taking Longsha Zhuzi Prescription,the patients continued to take the antiviral treatment combined Shenling Fuzheng Capsule,observing half a year.The clinical treatment efcet,the score changes of TCM syndromes(fever,cough,fatigue,indiges-tion and loss of appetite,darrhea,vomiting,etc.)before and after treatment,the changes of T elll subgroup indexes(CD_(4)^(+)_(4)^(+),CD_(4)^(+)_(4)^(+)/CD_(4)^(+)),quality of life score,adverse reactions and patient satisfaction were compared.The patients were fllowed up for 6 months to compare the incidence of opportunistic infection between the two groups.Results Compared with that(76.67%,23/30)of the control group,the total effective rate(96.67%,29/30)of observation group was higher(P<0.05).After treatment,the scores of TCM syndromes(fever,cough,fatigue,stupidity,diarrhea,vomiting,etc.),the scores of quality of life,and T cell subgroup indexes(CD_(4)^(+)_(4)^(+),CD_(4)^(+)_(4)^(+)/CD_(4)^(+)_(4)^(+))were improved in each group.The scores of TCM syndromes(fever,cough,fatigue,indigestin and loss of appetite.,diarrhea,vomiting,ete.)and the scores of quality of life in the observation group were better than those in the control group(P<0.05).However,there was no dfference in T cell subsets(CD_(4)^(+)_(4)^(+),CD_(4)^(+)_(4)^(+)/CD_(4)^(+))between the two groups afer treatment(P>0.05).There were no adverse reactions in both groups(P>0.05).The improvement rale of
