补肾活血汤联合芬吗通对薄型子宫内膜患者子宫内膜厚度及子宫动脉血流指数的影响

摘　　要：目的评价补肾活血汤联合芬吗通对薄型子宫内膜患者子宫内膜厚度及子宫动脉血流指数的影响。方法回顾性分析2018年1月1日—2018年12月31日在医院进行治疗的80例薄型子宫内膜患者临床资料,根据治疗方法的不同分为对照组(芬吗通联合阿司匹林治疗,39例)和观察组(对照组基础上+补肾活血汤,41例)。对照组在月经第5天开始给予芬吗通联合阿司匹林治疗,治疗期间若妊娠则停药,共治疗3个月。观察组在对照组治疗的基础上,加口服补肾活血汤治疗,治疗时间为3个月。比较两组患者治疗前后中医症候积分和治疗疗效,比较两组患者治疗前后子宫内膜厚度和成熟卵泡平均直径,比较两组治疗前后子宫血流动力学指标和1年内妊娠率。结果观察组治疗疗效较对照组高(P<0.05);两组治疗后中医症候积分低于治疗前,观察组下降幅度优于对照组(P<0.05);两组治疗后子宫内膜厚度和成熟卵泡平均直径均较治疗前增加,观察组增加幅度优于对照组(P<0.05);两组治疗后子宫血流动力学指标PI、RI和S/D较治疗前显著降低,观察组降低幅度优于对照组(P<0.05);观察组患者治疗后1年内妊娠率高于对照组(P<0.05)。结论补肾活血汤联合芬吗通治疗薄型子宫内膜疗效确切,可有效的改善临床症状,增加子宫内膜厚度,促进卵泡发育,降低子宫血流阻力指数,调节子宫的血液循环,提高子宫内膜容受性,提高胚胎着床率和妊娠率,值得临床进一步研究探讨。Objective To evaluate the efet of Bushen Huoxue Decoction(补肾活血汤)combined with Femotome on endome-trial thickness and uterine artery blood flow index in patients with thin endomtrium.Methods The clinical data of 80 patients with thin endometrium trealed in our hospital from January I,2018 to December 31,2019 was retrospectively analyzed.Accord-ing to different treatment methods,they were divided into control group(Femotome combined with aspirin,39 cases)and obser-vation group(self-designed Bushen Huoxue Decoction,41 cases).The control group was given estradiol tablets/estradiol tripro-gesterone tablets on the 5th day of menstruation(I tableV/time,1 tableV/day).The drug was stopped during menstruation,and the drug was stopped in case of pregnancy during treatment(3 menstrual cycles in total).On the basis of treatment in the control group,the observation group used Bushen Huoxue Decoction orally.The duration of treatment was 3 menstrual eycles.The scores of TCM syndromes and therapeutic effcts were compared between the two groups before and after treatment,the endome-trial thickness and the average diameter of mature follicles were compared between the two groups before and after treatment,and the uterine hemodynamie indexes and pregnancy rate within 1 year were compared between the two groups before and after treat-ment.Results The therapeutic effect of the observation group was higher than that of the control group(P<0.05).After treat-ment,the score of TCM syndromes in the two groups was lower than that before treatment,and the decrease rate in the observa-tion group was better than that in the control group(P<0.05).Endometrial thickness and average diameter of mature fllicles in both groups were increased afer treatment,and the increase amplitude in the observation group was better than that in the control group(P<0.05).After treatment,the uterine hemodynamic indexes(PI,RI and S/D)in the two groups were significantly re-duced compared with those before treatment,and the reduction in the observation grou
