自拟消肿止痛汤熏洗对痔瘘术后疼痛及创面愈合的影响分析

摘　　要：目的探讨自拟消肿止痛汤熏洗对痔瘘术后疼痛及创面愈合的影响。方法选取2021年1月—2022年6月在我院行痔瘘术的患者80例为研究对象,按照随机数字表法将其分为观察组和对照组,各40例。对照组给予常规护理,观察组在对照组的基础上给予自拟消肿止痛汤熏洗干预,2组疗程均为10 d,并随访至创面愈合。比较2组患者术后疼痛评分、肛缘水肿评分、创面分泌物评分、首次排便时间、肛门坠胀消失时间、新生上皮出现时间、创面愈合时间、并发症发生情况以及临床疗效。结果观察组患者术后第10 d疼痛评分、肛缘水肿评分以及创面分泌物评分均低于对照组(P<0.05);观察组患者首次排便时间、肛门坠胀消失时间、新生上皮出现时间以及创面愈合时间均短于对照组(P<0.05);观察组患者并发症发生率为22.50%,低于对照组的52.50%(P<0.05);观察组患者治疗总有效率为82.50%,高于对照组的62.50%(P<0.05)。结论自拟消肿止痛汤熏洗治疗痔瘘术后患者,能够明显减轻患者术后疼痛,缓解肛缘水肿,加快创面愈合,减少并发症发生率,临床疗效显著,值得临床推广应用。Objective To investigate the effect of fumigation and washing with self-made Xiaozhong Zhitong Decoction on postoperative pain and wound healing of patients with hemorrhoid and fistula.Methods A total of 80 patients undergoing hemorrhoid and fistula operation in Dongting Branch of People’s Hospital of Xishan District from January 2021 to June 2022 were randomly assigned into an observation group and a control group according to random number table method,with 40 patients in each group.The control group received routine nursing,and the observation group received fumigation and washing with self-made Xiaozhong Zhitong Decoction on the basis of the therapy in the control group.The treatment course of both groups was 10 days,and the follow-up lasted until wound healing.Postoperative pain score,perianal edema score,wound secretion score,the time to first defecation,the time to disappearance of anal bulge,the time to emergence of new epithelia,the time to wound healing,incidence of complications,and clinical efficacy were compared between the two groups.Results On day 10 after surgery,the pain score,perianal edema score,and wound secretion score in the observation group were lower than those in the control group(P<0.05).The time to first defecation,the time to disappearance of anal bulge,the time to emergence of new epithelia,and the time to wound healing in the observation group were shorter than those in the control group(P<0.05).The incidence of complications in the observation group was 22.50%,lower than that(52.50%)in the control group(P<0.05).The total effective rate in the observation group was 82.50%,which was higher than that(62.50%)in the control group(P<0.05).Conclusion Self-made Xiaozhong Zhitong Decoction can significantly relieve postoperative pain,relieve perianal edema,speed up wound healing,reduce the incidence of complications,and has significant clinical efficacy in the patients with hemorrhoid and fistula,which is worthy of clinical application.
