超声引导下颌下腺穿刺在诊断IgG4相关性疾病中的应用

摘　　要：目的 探讨超声引导下颌下腺穿刺诊断在IgG4(免疫球蛋白G4)相关性疾病诊断中的应用。方法 选取就诊首都医科大学附属北京潞河医院并经超声引导下颌下腺穿刺病理证实的IgG4相关疾病患者17例,男12例,女5例,年龄46~82岁,平均(62.7±6.3)岁,对其血清结果、超声影像表现及穿刺结果进行探讨。结果 17例患者血清IgG4平均浓度为(17.2±12.8)g/L(2.38~39.8 g/L)。超声诊断颌下腺病变弥漫型12例(70.6%),局灶型5例(29.4%)。颌下腺穿刺术中及术后均未出现出血、感染、导管损伤等并发症。结论 对怀疑IgG4相关疾病患者,超声引导下颌下腺穿刺活检较为简单易行,对临床诊断与鉴别并及时采用激素治疗有着积极的帮助。Objective To evaluate the value of ultrasound-guided submandibular gland puncture in the diagnosis of IgG4(immunoglobulin G4)related diseases. Methods 17 patients with IgG4 related diseases who visited Beijing Luhe Hospital Affiliated to Capital Medical University and pathologically confirmed by ultrasound-guided submandibular gland puncture were enrolled, 12 were male and 5 were female, aged 46-82 years, with an average of(62.7±6.3) years. The serum results、ultrasonic imaging findings and puncture results of these cases were analyzed. Results Among the 17 patients, the average serum IgG4 concentration was(17.2±12.8) g/L(2.38-39.8 g/L). 12 cases(70.6%) were diffuse type and 5 cases(29.4%) were focal type diagnosed by ultrasound. There were no complications such as bleeding, infection and catheter injury during and after submandibular gland puncture. Conclusions For patients with suspected IgG4 related diseases, ultrasound-guided submandibular gland biopsy is relatively simple and easy, which is helpful for clinical diagnosis, differentiation, and timely hormone treatment.
