摘 要:目的 探讨超声辐照微泡促进骨髓间充质干细胞(MSCs)对压力性尿失禁(SUI)模型大鼠病理损伤的修复作用。方法 30只雌性SD大鼠随机分为5组,分别为空白对照组(C)、SUI造模组(SUI)、SUI+MSCs组(MSCs)、SUI+超声微泡组(USMB)、SUI+MSCs+超声微泡组(MSCs+USMB),每组6只。SUI组采用阴道扩张加卵巢切除法建立产伤SUI模型。MSCs组在造模后120 h尾静脉注入MSCs 0.5 mL。USMB组在造模后72 h、120 h注入造影剂0.5 mL同时启动超声辐照2 min。MSCs+USMB组除USMB处理外,在造模后120 h尾静脉注入MSCs 0.5 mL。治疗后7 d进行尿流动力学检测以及对尿道组织取材切片,评估尿道括约肌损伤程度和恢复水平。结果 尿流动力学检测显示SUI组大鼠漏尿点压力(LPP)明显下降,膀胱容积(BC)明显缩小。和SUI组相比,接受MSCs+USMB治疗大鼠LPP和BC明显升高(P<0.05),而单纯接受MSCs或USMB治疗大鼠的LPP和BC无明显恢复(P>0.05)。MHC和α-SMA免疫组化和荧光提示,接受MSCs+USMB治疗大鼠的尿道括约肌恢复好于SUI组(P<0.05),而单纯接受MSCs或USMB治疗大鼠的尿道括约肌无明显恢复(P>0.05)。结论 超声联合微泡可促进MSCs修复产伤性模型大鼠尿道组织,有希望成为一种新的无创性治疗SUI途径。Objective To explore the repair effect of mesenchymal stem cells(MSCs) under ultrasound combined microbubble promotes on stress urinary incontinence in rats. Methods Thirty female SD rats were randomly divided into the following 5 groups, including control group(C), SUI group(SUI), SUI+MSCs group(MSCs), SUI+ultrasound microbubble group(USMB), SUI+MSCs+ultrasound microbubble group(MSCs+USMB), with 6 rats in each group. In the SUI group, vaginal dilation and ovariectomy were performed to establish the stress urinary incontinence model mimicking birth injury. In the MSCs group, 0.5 mL of MSCs was injected into caudal vein 120 hours after modeling. In the USMB group, 0.5 mL of microbubbles was injected into the caudal vein when the animals were treated by ultrasound for 2 minutes at 72 hours and 120 hours after modeling. In the MSCs+USMB group, 0.5 mL of MSCs was injected into caudal vein 120 hours after modeling except for USMB treatment. On 7 days after treatment, urine flow dynamics were evaluated. Then rats were sacrificed and the urethral tissues were collected. Results Urine flow dynamics showed that leak point pressure(LPP) and bladder capacity(BC) in SUI group were significantly decreased. Compared with SUI group, LPP and BC of rats treated with MSCs+USMB were significantly increased(P<0.05). LPP and BC of rats treated with MSCs or USMB alone did not recover significantly(P>0.05). Immunohistochemistry and immunofluorescence of MHC and α-SMA indicated that the urethral muscle of the rats treated with MSCs+USMB r was better than that of the SUI group(P<0.05). But there was no significant recovery of the urethral muscle in rats treated with MSCs or USMB alone(P>0.05). Conclusions Ultrasound combined with microbubbles can promote MSCs to repair the urethral tissue of SUI rat model, which is a potential new non-invasive approach for the treatment of SUI.
