摘  要:目的 探讨IgG4相关性疾病的超声表现,提高对该病的认识和诊断水平。方法 回顾性收集21例IgG4相关性疾病的临床资料及复习相关文献,分析其超声表现特点。结果 21例患者共37个器官累及,13例为单器官病变,3例2器官累及,3例3器官累及,1例4器官累及,1例5器官累及。胰(7例)、胆管(6例)、肝(5例)、肾(3例)、涎腺(8例)、泪腺及眼眶(2例)受累表现可分为弥漫性病变和局灶性病变。甲状腺(2例)受累表现为甲状腺局部见低回声结节,边界欠清晰。淋巴结(4例)受累表现为边界清楚,内部回声减低,血流信号较丰富,淋巴门样结构存在,未见钙化及液化。随访2例糖皮质激素治疗前后超声图像比较:低回声结节较前缩小或消失。结论 IgG4相关性疾病的超声表现为弥漫性和局灶性病变,可多器官或单器官累及,确诊需要病理诊断;提高对该病的认识和诊断水平对临床诊治及随访具有重要意义。Objective To explore the ultrasonic features of IgG4 related diseases and improve the understanding and accuracy of diagnosis of the disease. Methods The clinical data of 21 patients with IgG4 related diseases were collected retrospectively, and the related literatures were reviewed. The ultrasonic features were analyzed. Results There were totally 37 lesions in the 21 cases, of which 8 cases were multiple organs involved and 13 cases were single organ involved. The involvement of pancreas(7 cases), bile duct(6 cases), liver(5 cases), kidney(3 cases), salivary gland(8 cases), lacrimal gland and orbit(2 cases) could be appeared as diffuse and focal lesions. Thyroid involvement(2 cases) showed hypoechoic nodules with unclear boundaries. The involved lymph nodes(4 cases) showed clear boundary, decreased internal echo, abundant blood flow signals, existence of lymphatic hilus like structures, and no calcification and liquefaction. The ultrasonic images of 2 cases were compared before and after glucocorticoid treatment: the hypoechoic nodules were smaller or disappeared than before. Conclusions Lesions of IgG4-related diseases can be either diffuse or focal distribution in ultrasonic images, involving multiple organs or single organ. It’s important for clinical diagnosis and treatment to improve the comprehension of IgG4-related diseases and the diagnostic ability of ultrasound.
