摘 要:目的:探讨三维重建技术在腹腔镜肝尾状叶切除术(LCH)中的应用效果。方法:前瞻性纳入2018年1月至2020年12月72例行LCH患者的临床资料,采用随机数字表法将患者分为对照组和研究组,每组各36例。对照组行LCH,研究组联合三维重建技术行LCH。数据应用软件SPSS 22.0分析,围手术期相关指标、肝功能指标等计量资料采用(x±s)表示,行独立样本t检验;术后并发症等计数资料行χ%(2)检验。P<0.05表示差异有统计学意义。结果:研究组手术时间、术中出血量、术中输血量、术后排气时间及术后住院时间较对照组显著降低(P<0.05)。术后各时间点肝功能指标与术前比较差异均有统计学意义(P<0.05),且研究组显著优于对照组(P<0.05)。研究组患者术后并发症的总发生率较对照组显著降低(11.1% vs. 30.6%,P<0.05)。结论:LCH术联合应用三维重建技术不仅能够有效的缩短手术时间,减少术中损伤,加快患者术后恢复,而且也有利于术后肝功能的恢复,降低患者术后并发症的发生,提高手术安全性。Objective To investigate the eflet of the-dimenional reconstruetion in laparoscopice ceudate lobectomy(LCH).Methods The elinical daa of 72 patients with LCH from January 2018 to December 2020 were propetively ineluded.The patients were divided into control group and study group by random mumber table method,wit 36 cases in each group.The contol group underwent LCH,and the study goup combined with 3D reonstruetion tehnoloy underwent LCH.SPSS 22.0 was used to analye the data.Perioperaive idicators,liver function indicators and other measurenment data were expresed as(x±s),and independent sample t test was performed.Postoperative complications and other counting dala were analyed byx test.P<0.05 idicates saistcal sgificance.Results The operution time.intaperatire bood lss.s intaperaive blood tasfusin,postoperative exhaust time and postoperative hopital stay in the study group were sgificanly lower than those in the eontrol group(P<0.05).There were sgnifcant dferece in liver function indexe beween the two groups(P<0.05),and the study goup was sinicanly better than the control goup(P<0.05).The total ineidence of postoperative complcations in the study group was sigifeanly lower than that in the conrol group(11.1%vs.30.6%.P<0.05).Conclusion LCH combined with treedimensional reconstruetion technology can not only eftiell shorten the operation time,reduce itraperative injury,aceleate the potoperatve rcovery of patients,but also is conducive to the reovery of pstpantive liver function,reduee the ocurence of potoperative complications.and improve the safety of surgery.
