摘 要:目的探究血管生成素-2(Angiopoietin-2,Ang-2)、尿基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)、微小RNA-142-3p(microRNA-142-3p;miR-142-3p)在脓毒症合并急性肾损伤(acute kidney injury,AKI)患者中的表达以及相关性分析研究。方法选取2020年7月至2021年7月进行收治的脓毒症合并AKI患者60例,AKI组30例,非AKI组30例,另选30组健康志愿者为对照组。酶联免疫吸附实验法检测Ang-2、MMP-9、超氧化物歧化酶(superoxide dismutase,SOD)、丙二醛(malondialdehyde,MDA)、总抗氧化能力(total antioxidant capacity,T-AOC)水平;RT-PCR法检测miR-142-3p表达,比较3组患者Ang-2、MMP-9、miR-142-3p表达及SOD、MDA、T-AOC水平;进一步分析三者的相关性及对脓毒症合并AKI的诊断价值。结果与对照组、非AKI组比较,AKI组Ang-2、MMP-9、miR-142-3p表达、MDA水平升高(P<0.05);SOD、T-AOC水平降低(P<0.05);随着病情严重程度、APACHEⅡ评分及Scr水平情况,Ang-2、MMP-9、miR-142-3p表达升高(P<0.05)。Ang-2表达与Cys-C、MDA水平呈正相关(P<0.05),与eGFR、SOD、T-AOC水平呈负相关;MMP-9表达与Cys-C、MDA水平呈正相关(P<0.05),与eGFR、SOD、T-AOC水平呈负相关(P<0.05);miR-142-3p表达与Cys-C、MDA水平呈正相关(P<0.05),与eGFR、SOD、T-AOC水平呈负相关(P<0.05)。脓毒症合并AKI患者Ang-2、MMP-9表达呈正相关(r=0.502,P=0.005);脓毒症合并AKI患者MMP-9、miR-142-3p表达呈正相关(r=0.543,P=0.001);脓毒症合并AKI患者Ang-2、miR-142-3p表达呈正相关(r=0.489,P=0.006)。结论Ang-2、MMP-9、miR-142-3p表达在脓毒症合并AKI患者中存在异常,三者表达变化与脓毒症合并AKI患者病理特征、肾功能、氧化应激反应指标具有相关性,且Ang-2、MMP-9、miR-142-3p表达之间具有一定相关性,三者联合对脓毒症合并AKI诊断价值更高。Objective To investigate the expressions of angiopoietin-2(Ang-2),urinary matrix metalloproteinase-9(MMP-9)microrna-142-3p(microRNA-142-3p,miR-142-3p)in patients with sepsis complicated by acute kidney injury(AKI),and to explore its correlation.Methods Sixty patients with sepsis complicated by AKI who were treated in our hospital from July 2020 to July 2021 were enrolled in the study,who were divided into AKI group and non AKI group,with 30 cases in each group,at he same time,the other 30 volunteers were enrolled as control group.The expression levels of Ang-2,MMP-9,superoxide dismutase(SOD),malondialdehyde(MDA)and total antioxidant capacity(T-AOC)were detected by enzyme-linked immunosorbent assay,and the expression levels of miR-142-3p were detected by RT-PCR.The expression levels of Ang-2,MMP-9,miR-142-3p and the levels of SOD,MDA and T-AOC were observed and compared among the three groups.Moreover the correlation among the indexes and the diagnostic value of the three indexes in diagnosis of sepsis complicated by AKI was analyzed.Results Compared with those in control group and non AKI group,the expression levels of Ang-2,MMP-9,miR-142-3p and MDA levels in AKI group were significantly increased(P<0.05),however,the levels of SOD and T-AOC were significantly decreased(P<0.05).Moreover the expression levels of Ang-2,MMP-9 and miR-142-3p were increased with the severity of the disease,ApacheⅡscore and SCR level(P<0.05).In addition the expression levels of Ang-2 were positively correlated with those of Cys-C and MDA,however,which were negatively correlated with those of EGFR,SOD and T-AOC.And the expression levels of MMP-9 were positively correlated with those of Cys-C and MDA,however,which were negatively correlated with those of EGFR,SOD and T-AOC.The expression levels of mir-142-3p were positively correlated with the levels of Cys-C and MDAbut,which were negatively correlated with the levels of EGFR,SOD and T-AOC.There was a positive correlation between Ang-2 and MMP-9 in patients with sepsis complicated by AK
