摘  要:目的 评估骨水泥强化空心侧孔螺钉固定技术治疗骨质疏松性胸腰椎骨折的疗效与安全性。方法 将我院脊柱外科2018年4月~2021年4月收治的90例骨质疏松性胸腰椎骨折患者作为本次研究对象,按照随机数字表法将其分为骨水泥组(n=45)和传统组(n=45),骨水泥组采用骨水泥强化空心侧孔螺钉固定技术,传统组采用传统椎弓根螺钉固定技术,比较并发症发生情况、腰椎功能、椎体恢复情况。结果 骨水泥组术后并发症发生率低于传统组,两组比较差异无统计学意义(P>0.05);两组治疗后腰椎功能(JOA)评分均呈显著上升趋势(P<0.05),且骨水泥组术后1d、术后1个月、术后3个月的JOA评分均显著高于传统组(P<0.05);术后3个月,骨水泥组Cobb角小于传统组,后缘、中线以及前缘高度均显著高于传统组(P<0.05)。结论 骨水泥强化空心侧孔螺钉固定技术治疗骨质疏松性胸腰椎骨折能够尽快改善腰椎功能,且具备安全性。Objective To evaluate the efficacy and safety of bone cement augmented fenestrated pedicle screw technique in the treatment of osteoporotic thoracolumbar fractures.Methods A total of 90 patients with osteoporotic thoracolumbar fractures who were treated in department of spine surgery of the hospital from April 2018 to April 2021 were selected as the research subjects,and they were divided into bone cement group(n=45)and traditional group(n=45)according to the random number table method.The bone cement group adopted bone cement augmented fenestrated pedicle screw technique while the traditional group used traditional pedicle screw fixation technique.The occurrence of complications,lumbar function,and vertebral body recovery were compared.Results The incidence rates of postoperative complications in bone cement group were lower than those in traditional group(P>0.05).After treatment,the score of lumbar function(Japanese Orthopedic Association,JOA)in both groups showed a significant upward trend(P>0.05),and the JOA scores in bone cement group at 1 day,1 month,and 3 months after surgery were significantly higher compared with those in traditional group(P<0.05).At 3 months after surgery,the Cobb angle in bone cement group was smaller than that in traditional group,and the heights of posterior margin,midline and anterior margin were significantly higher than those in traditional group(P<0.05).Conclusion Bone cement augmented fenestrated pedicle screw technique in the treatment of osteoporotic thoracolumbar fractures can improve lumbar function as soon as possible,and it has safety.
