基于明清时期小儿古籍文献考析小儿之“筋”

摘　　要：“筋”乃中医理论中的概念,属“五体”范畴,此概念在推拿临床治疗中至关重要。以明清时期具有代表性的小儿古籍(包括《秘传看惊掐筋口授手法论》《小儿按摩经》《小儿推拿秘旨》《厘正按摩要术》《幼科推拿秘书》《幼科铁镜》《幼幼集成》)为文献来源,对“筋”的含义、类型、分布规律、诊疗价值进行归纳梳理,发现“筋”在小儿古籍中有静脉、穴位名称、五体之一、联络关节肌肉的组织含义,并在此基础上对六筋理论进行分析,阐释六筋与部分穴位及手法的内在联系及理论意义。同时,对“六筋”指导中医辨证及具有调阴阳、和脏腑、镇惊止痉的作用进行总结,旨在为“筋”这一中医概念更好地指导小儿推拿临床提供参考。"Sinew"is a traditional Chinese medicine(TCM)concept,which belongs to the“five-body constitu-ents”category and is crucial in the clinical treatment with Tui na.The meaning,types,distribution law and diagnosis and treatment value of“sinew”were summarized by reviewing representative ancient pediatric books in Ming and Qing dynasties,including Secret Transmissions of Manipulation of Managing Frightening and Pinching Sinews(《秘传看惊掐筋口授手法论》),Classic of Children Massage(《小儿按摩经》),Secrets of Children Tuina(《小儿推拿秘旨》),Cor⁃rected Massage Essentials(《厘正按摩要术》),Secret Book on Pediatric Tuina(《幼科推拿秘书》),Iron Mirror on Pediat⁃rics(《幼科铁镜》)and The Grand Compendium of Pediatrics(《幼幼集成》).It is found that"sinew"has the meaning of veins,acupoints,one of the five body constitutions,and tissue connecting joints and muscles in the ancient pediatric books.On this basis,the theory of six sinews was analyzed,and the inner connection and theoretical significance of the six sinews and certain acupoints and manipulations were explained.Simultaneously,this paper summarized the functions of six sinews in guiding syndrome differentiation in TCM as well as in regulating yin and yang,harmonizing zang-fu(脏腑)organs,suppressing fright and arresting convulsion,with the aim of using the concept of“sinew”to better guide the clinical practice of pediatric tuina.
