MRI扩散加权定量参数ADC值与前列腺癌Gleason评分及血清PSA相关性分析

摘　　要：目的分析核磁共振(MRI)扩散加权定量参数常规表观扩散系数(ADC)值与前列腺癌Gleason评分及血清前列腺特异性抗原(PSA)相关性。方法选取2017年11月至2019年11月本院收治的62例前列腺癌患者作为研究对象,所有患者均经活检穿刺证实为前列腺癌,术前均进行MRI和血清PSA检查,根据Gleason评分将患者分为高分化组(n=9例,2~4分)、中分化组(n=32例,5~7分)和低分化组(n=21例,8~10分),比较不同病理分组患者ADC值及tPSA、fPSA水平,采用Spearman分析ADC值与Gleason评分及tPSA、fPSA的相关性。结果随着分化程度的降低,ADC值逐渐降低,tPSA、fPSA逐渐升高,三组之间比较差异具有统计学意义(P<0.05);经Spearman相关性结果分析显示,ADC值与Gleason评分和tPSA、fPSA成负相关(P<0.05);直线相关散点图显示,ADC值与Gleason评分和tPSA、fPSA存在直线相关性(R2=0.832,P<0.05;R 2=0.381,P<0.05;R 2=0.492,P<0.05)。结论小视野前列腺ADC图可提高肿瘤的可见率,与Gleason评分及血清PSA均具有较好的相关性,为临床鉴别前列腺癌及病理分级提供一定参考的依据。Objective To analyze the correlation among the quantitative parameter conventional apparent diffusion coefficient(ADC)value of MRI diffusion weighted imaging,prostate cancer Gleason score and serum prostate specific antigen(PSA).Methods Between November 2017 and November 2019,62 patients with prostate cancer confirmed by biopsy were enrolled in the study.MRI and serum PSA detection were performed before surgery.According to the Gleason score,the patients were divided into high differentiation group(9 cases,2~4 points),moderate differentiation group(32 cases,5~7 points)and poor differentiation group(21 cases,8~10 points).The ADC values,tPSA and fPSA levels of the 3 groups were compared,and Spearman correlation analysis of ADC value with Gleason score,tPSA and fPSA was performed.Results As the differentiation degree decreased,the ADC value decreased,tPSA and fPSA increased,indicating statistically significant differences among the three groups(P<0.05).Spearman correlation analysis showed that the ADC value was negatively correlated with Gleason score,tPSA and fPSA(P<0.05).The rectilinear correlation scatter plot showed that there was a linear correlation between ADC value and Gleason score,tPSA and fPSA(R2=0.832,P<0.05;R2=0.381,P<0.05;R2=0.492,P<0.05).Conclusion The small field-of-view prostate ADC image can improve the visibility of tumors,and it is associated with Gleason score and serum PSA.It can provide reference for clinical diagnosis of prostate cancer and pathological grading.
