成人不同重组乙型肝炎疫苗接种后18年免疫记忆持久性分析

摘　　要：目的观察成人不同重组乙型肝炎疫苗全程接种18年后免疫记忆情况。方法应用队列研究的方法,选择重组乙肝疫苗(CHO细胞)20μg、10μg和重组乙肝疫苗(酵母)10μg采用0、1、6个月免疫程序后免疫成功的成人作为研究对象,并于免疫后18年检测血清中HBsAg、抗-HBs和抗-HB水平。结果接种CHO细胞20μg、CHO细胞10μg和酵母10μg成人18年后,抗-HBs阳性率分别为86.67%、62.32%、57.14%,差异有统计学意义(χ^(2)=16.66,P<0.01);抗-HBs几何平均浓度(GMC)分别为1268.47、118.51、209.27 mIU/ml,组间差异有统计学意义(F=20.40,P<0.01)。CHO细胞20μg组女性的抗-HBs阳性率高于男性(χ^(2)=14.16,P<0.01);CHO细胞20μg>35岁年龄组抗-HBs阳性率高于其他2组(χ^(2)=8.27,P=0.02)。酵母10μg组发现2例HBsAg阳性,阳性率为3.17%。结论接种重组乙型肝炎疫苗(CHO)20μg成人18年后抗-HBs依然维持较高水平,免疫持久性效果较好,推荐成人优先接种该疫苗。Objective To observe the immune memory persistence among adults 18 years after vaccination with different recombinant hepatitis B vaccines.Methods Using the cohort study method,the subjects vaccinated with 20μg/10μg recombinant hepatitis B vaccine(CHO cell)and 10μg recombinant hepatitis B vaccine(yeast)of 0,1,6 months immunization program were selected,whose serum HBsAg,anti-HBs and anti-HBc levels were detected at 18 years after immunization.Results After 18 years of inoculation with 20μg/10μg CHO cells and 10μg yeast vaccines,the positive rates of anti-HBs were 86.67%,62.32%and 57.14%,respectively,with statistically significant difference(χ^(2)=16.66,P<0.01);the geometric mean concentrations(GMC)of anti-HBS were 1268.47,118.51 and 209.27,respectively,with statistically difference(F=20.40,P<0.01).Among subjects inoculated with 20μg CHO cell vaccines,the anti-HBs positive rate was significantly higher in female than in that in male(χ^(2)=14.16,P<0.01);the anti-HBs positive rate of subjects aged>35 year was significantly higher than those in other 2 groups(χ^(2)=8.27,P=0.02).There were 2 HBsAg positive cases reported among subjects inoculated with 10μg yeast vaccines,resulting HBsAg positive rate of 3.17%.Conclusion After 18 years of inoculation with 20μg recombinant hepatitis B vaccines(CHO),the anti-HBs level maintains at high levels among adults,demonstrating good immune persistence effect.Adults are recommended to give priority to inoculate 20μg recombinant hepatitis B vaccines(CHO).
