摘 要:目的:研究在多普勒超声引导下颈神经根定位的正确率。方法:筛选40名志愿者进行研究。依据C_(7)横突前方的椎动脉和后结节,以及C_(5)、C_(6)横突有前后结节的解剖学特征,通过多普勒超声图像来辨认它们的位置。然后,将穿刺针分别放置在C_(5)、C_(6)和C_(7)横突上,并通过X光透视检查对比确认穿刺针的位置与方向是否正确。结果:在40名志愿者左右两侧共240个节段中,穿刺针能放置在正确解剖位置的概率是94.2%;穿刺针正确指向C_(5)、C_(6)横突上的概率均为95.0%;正确指向C_(7)横突上的概率是92.5%。结论:同时应用椎动脉和C_(5~7)横突形态特征作为多普勒超声图像的两个重要解剖标记,以确定目标颈椎水平的正确率为94.1%。因此,多普勒超声引导下的颈神经根定位是安全、实用、可靠的。Objective:To investigate the accuracy rate of cervical nerve root localization under Doppler ultrasound-guidance.Methods:For the study,40 volunteers underwent screening.According to the anatomical features of the vertebral artery in front of C_(7) transverse process with only posterior tubercle,C_(5),C_(6) with anterior and posterior tubercle of transverse process,their positions were identified by Doppler ultrasound images.Subsequently,the puncture needles were placed on the C_(5),C_(6) and C_(7) transverse processes,respectively.And through X-ray fluoroscopy to confirm whether the position and direction of the metal needle are correct.Results:In a total of 240 segments on the left and right sides of 40 volunteers,the probability that the puncture needle can be placed in the correct anatomical position is 94.2%.The probability of metal needle accurately pointing to C_(5),C_(6) transverse process is 95.0%;the probability of accurately pointing to C_(7) transverse process is 92.5%.Conclusion:Simultaneous use of vertebral artery and C_(5~7) transverse process morphological features as two important anatomical markers in Doppler ultrasound images,the accuracy of determining the target cervical level is 94.1%.Therefore,Doppler ultrasound-guided cervical nerve root localization is safe,practical and reliable.
