摘　　要：目的:探讨血清S-100β与神经元特异性烯醇酶(NSE)水平在老年髋关节置换术后谵妄(PD)患者中的临床意义.方法:选择本院行全麻下髋关节置换术的120例老年患者为研究对象.采用意识模糊评估法(CAM)诊断患者PD.将术后持续2d发生谵妄者纳入谵妄组(26例),另选取未发生谵妄的患者26例纳入非谵妄组.分别在麻醉前、术毕、术后1d和2d检测两组患者的血清S-100β蛋白与NSE水平.结果:两组患者的一般资料比较,差异无统计学意义(P>0.05).与麻醉前及非谵妄组比较,谵妄组术毕、术后1d和2d的血清S-100β蛋白水平均升高(P<0.05).两组患者各时间点血清NSE水平的差异均无统计学意义(P>0.05).结论:血清S-100β蛋白水平能够作为老年髋关节置换PD的监测指标之一,可用于临床早期预测PD发生.Objective:To investigate the clinical significance of serum S-10oβprotein and neuron-specific enolase(NSE)levels in elderly patients with delirium after hip joint replacement.Methods:A total of 120 elderly patients who underwent hip replacement under general anesthesia in our hospital were selected as study subjects.Their postoperative delirium(PD)were diagnosed by confusion assessment method(CAM).The patients who had delirium for 2d after operation were included in the delirium group(26 cases),and the other 26 patients who did not have delirium were selected as the non-delirium group.The levels of serum S-100βprotein and NSE of the two groups of patients were detected before anesthesia,at the end of operation,1 and 2d after operation.Results:There was no significant dfference between the two groups in general data(P>0.05).Compared with the pre-anesthesia and non-delirium group,the level of serum S-100βprotein increased in the delirium group at the end of operation,1 and 2d after operation(P<0.05).There was no significant diference in serum NSE level between the two groups at each time point(P>0.05).Conclusion:The serum S-10oβprotein level can be used as one of the monitoring indicators of delirium after hip replacement in the elderly,and can be used to predict PD in early clinical stage.
