摘　　要：目的探讨三维立体定位穿刺与手术在乳腺微钙化灶患者中的应用价值。方法回顾性分析2020年9月至2021年12月上饶市妇幼保健院收治的75例乳腺微钙化灶患者的临床资料,术后病理检查结果为金标准,统计三维立体定位穿刺与手术与乳房钼靶检查结果灵敏度、特异度、准确率;评估三维立体定位穿刺与手术、乳房钼靶检查结果与手术病理检查结果一致性;绘制三维立体定位穿刺与手术诊断乳腺微钙化灶的受试者工作特征(ROC)曲线。结果三维立体定位穿刺与手术灵敏度、特异度、准确率均高于乳房钼靶检查结果;三维立体定位穿刺与手术与病理检查结果具有一致性(Kappa=0.551,P<0.05),乳房钼靶检查结果与手术病理检查结果不具有一致性(Kappa=0.085,P>0.05);绘制三维立体定位穿刺与手术诊断乳腺微钙化灶的ROC曲线,结果显示其曲线下面积为0.812。结论三维立体定位穿刺与手术有利于提高癌症筛查水平,减少漏诊、误诊发生,促进临床治疗,值得在临床推广应用。Objective To investigate the application value of three-dimensional stereotaxic puncture and surgery in patients with breast microcalcification.Methods The clinical data of 75 patients with breast microcalcification admitted to Shangrao Maternal and Child Health Hospital from September 2020 to December 2021 was retrospectively analyzed.All patients underwent three-dimensional stereotactic puncture and surgery.Postoperative pathological results were taken as the golden standard to analyze the sensitivity,specificity,and accuracy of three-dimensional stereotaxic puncture and surgery and mammography.The consistencies between the results of three-dimensional stereotaxic puncture and surgery and mammography and surgical pathological results were evaluated.The value of three-dimensional stereotaxic puncture and surgery in the diagnosis of breast microcalcification was analyzed using receiver operating characteristic(ROC)curve.Results The sensitivity,specificity and accuracy of three-dimensional stereotaxic puncture and surgery were higher than those of mammography.The results of three-dimensional stereotaxic puncture and surgery were consistent with surgical pathological results(Kappa=0.551,P<0.05),but the results of mammography were not consistent with surgical pathological results(Kappa=0.085,P>0.05).ROC curve analysis showed that the area under the curve of three-dimensional stereotactic puncture surgery to diagnose breast microcalcification was 0.812.Conclusion Three-dimensional stereotactic puncture and surgery is beneficial to improve the level of cancer screening,reduce missed diagnosis and misdiagnosis,and promote clinical treatment.
