摘  要:目的探讨3D打印技术联合S-ROM假体及转子下斜型截骨在全髋关节置换治疗CroweⅣ型成人髋关节发育不良中的应用效果。方法回顾性选取2015年3月至2020年4月福建医科大学附属宁德市闽东医院收治的40例CroweⅣ型成人髋关节发育不良行全髋关节置换患者作为研究对象,根据不同治疗方法将其分为观察组(28例)和对照组(12例)。观察组患者采用3D打印联合S-ROM假体及转子下斜型截骨技术,对照组患者单纯采用S-ROM假体及转子下斜型截骨技术。比较两组患髋手术前后的Harris评分、疼痛视觉模拟评分法(VAS)评分,手术时间、术中出血量,分析手术前后的临床及影像资料、术后转子下截骨愈合情况。结果术前,两组患者的髋关节Harris评分、VAS评分比较,差异无统计学意义(P>0.05);术后6个月,两组患者的髋关节Harris评分高于术前,VAS评分低于术前,差异有统计学意义(P<0.05)。观察组患者的手术时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。两组患者均恢复日常生活,转子下截骨均获得骨性愈合。结论对CroweⅣ型髋关节发育不良患者行全髋关节置换术时,手术难度较大,3D打印技术联合S-ROM假体及转子下斜型截骨能使手术简单化,缩短手术时间,减少术中出血量。Objective To investigate the effect of 3D printing technology combined with S-ROM prosthesis and oblique trochanteric osteotomy in the treatment of developmental dysplasia Crowe typeⅣwith total hip arthroplasty.Methods A total of 40 adult patients with Crowe typeⅣhip dysplasia who underwent total hip arthroplasty in Mindong Hospital of Ningde,Fujian Medical University from March 2015 to April 2020 were retrospectively selected as the research objects.According to different treatment methods,they were divided into observation group(28 cases)and control group(12 cases).The observation group was treated with 3D printing combined with S-ROM prosthesis and subtrochanteric oblique osteotomy,while the control group was treated with S-ROM prosthesis and subtrochanteric oblique osteotomy alone.Harris score,visual analogue scale(VAS)score before and after operation,operation time,intraoperative blood loss,clinical and imaging data and postoperative subtrochanteric osteotomy healing were compared between the two groups.Results Before operation,there were no significant differences in Harris score and VAS score of hip joint between the two groups(P>0.05).At 6 months after operation,the Harris scores of hip joint in the two groups were higher than those before operation,and the VAS scores were lower than those before operation,and the differences were statistically significant(P<0.05).The operation time of the observation group was shorter than that of the control group,and the intraoperative blood loss was less than that of the control group,the differences were statistically significant(P<0.05).Both groups of patients returned to daily life,and bone healing was achieved in subtrochanteric osteotomy.Conclusion Total hip replacement for Crowe typeⅣdysplasia of the hip is difficult to perform.3D printing technology combined with S-ROM prosthesis and subtrochanteric oblique osteotomy can simplify the operation,shorten the operation time and reduce the amount of intraoperative blood loss.
