住院患者对“一患一陪护”制度内心感受的质性研究

摘　　要：目的探讨住院患者对“一患一陪护”制度的内心感受,为进一步改进陪护制度提供参考。方法采用目的性抽样法,选取2021-02~03月在武汉市某三级甲等医院耳鼻咽喉头颈外科住院患者进行半结构式深入访谈,以受访者资料不再出现新的主题作为标准,共访谈10名患者,并用Colaizzi 7步分析法分析资料。结果共提炼出6个主题,其中积极主题3个,包括病室环境安静、治疗周期缩短且疗效明显、医护患关系和谐;消极主题3个,分别是感到孤独、需要更多的帮助与支持、心情烦躁。结论后疫情时代,“一患一陪护”制度对住院患者产生了一定的影响,针对消极方面,建议在临床工作中依据科室特色完善人性化的陪护制度、提升医务人员的人文关怀能力以及提供娱乐设施及场所的措施,以促进形成良好的医护患关系,满足住院患者身心健康需求,提升诊疗效果及满意度。Objective To explore the inner feelings of inpatients to the"one patient one escort"system,so as to provide reference for further improvement of companion system.Methods Patients hospitalized in department of otolaryngology-head and neck surgery in a class A tertiary hospital in Wuhan from February to March 2021 were given semi-structured interview by purposive sampling method.A total of 10 patients were interviewed and analyzed by Colaizzi 7-step analysis method based on the criterion that no new topics appeared in the data of the interviewees.Results A total of 6 themes were refined,of which 3 were positive themes,including quiet ward environment,shortened treatment cycle and obvious curative effect and harmonious relationship between doctors and nurses;3 were negative themes,namely feeling lonely,needing more help and support and feeling irritable.Conclusion In the post-epidemic era,"one patient one escort"system has a certain impact on inpatients.In view of the negative aspects,it is suggested to improve the humanized nursing system according to the characteristics of the department,improve the humanistic care ability of medical personnel and provide entertainment facilities and places in clinical work in the future,so as to promote the formation of a good doctor-patient relationship,meet the physical and mental health needs of hospitalized patients and improve its diagnosis and treatment effects and the degree of satisfaction.
