改良后路腰椎椎体间融合术治疗腰椎间盘突出症的效果

摘　　要：目的探讨改良后路腰椎椎体间融合术治疗腰椎间盘突出症的效果。方法回顾性收集邓州市中心医院2018年1月至2020年12月收治的接受改良后路腰椎椎体间融合术治疗的31例腰椎间盘突出症患者的临床资料,纳入改良组;另收集医院同期接受传统后路椎弓根螺钉固定术治疗的31例腰椎间盘突出症患者的临床资料,纳入传统组。比较两组手术指标(手术时间、术中出血量以及术后引流量)。比较术前、术后6个月腰椎功能[Oswestry功能障碍指数(ODI)]、术后6个月并发症发生情况以及椎体间融合情况。结果改良组手术时间短于传统组,术中出血量、术后引流量少于传统组(P<0.05);两组术后6个月ODI指数低于术前,且改良组低于传统组(P<0.05);术后两组并发症发生情况和椎体间融合度比较,差异无统计学意义(P>0.05)。结论与传统后路椎弓根螺钉固定术相比,改良后路腰椎椎体间融合术可缩短腰椎间盘突出症患者的手术时间,减少出血量,促进患者术后腰椎功能恢复。Objective To investigate the effect of improved posterior lumbar interbody fusion in the treatment of lumbar disc herniation.Methods The clinical data of 31 patients with lumbar disc herniation treated by improved posterior lumbar interbody fusion in Dengzhou Central Hospital from January 2018 to December 2020 were retrospectively collected and included in the improved group.In addition,the clinical data of 31 patients with lumbar disc herniation who received traditional posterior pedicle screw fixation in the same period in the hospital were collected and included in the traditional group.The operation indexes(operation time,intraoperative bleeding amount and postoperative drainage amount)were compared between the two groups.The preoperative and postoperative lumbar function[Oswestry disability index(ODI)],postoperative complications and interbody fusion were compared between the two groups before and 6 months after operation.Results The operation time in the improved group was shorter than that in the traditional group,and the amount of intraoperative bleeding and postoperative drainage in the improved group were less than those in the traditional group(P<0.05).The ODI index of both groups 6 months after operation was lower than that before operation,and the improved group was lower than that of the traditional group(P<0.05).There was no statistical difference in the incidence of complications and the degree of interbody fusion between the two groups(P>0.05).Conclusion Compared with the traditional posterior pedicle screw fixation,the improved posterior lumbar interbody fusion could shorten the operation time,reduce the amount of bleeding and promote the recovery of postoperative lumbar function in patients with lumbar disc herniation.
