摘　　要：目的 探讨血清视黄醇结合蛋白4(RBP4)及子宫内膜组织的RBP4、过氧化物酶增殖物激活受体γ(PPARγ)水平与子宫内膜增生的关系;分析血清RBP4水平和子宫内膜组织的RBP4、PPARγ表达水平的关系。方法 选取深圳市宝安区妇幼保健院生殖健康科2017年7月至2018年12月收治的异常子宫出血患者48例,根据病理分型分为A组(n=24):子宫内膜不伴有不典型的增生(单纯型增生或复杂型增生);B组(n=6):子宫内膜不典型增生或子宫内膜癌。对照组(n=18)为子宫内膜正常组(子宫内膜处于增生期或分泌期),检测患者血清RBP4水平及子宫内膜组织RBP4和PPARγ表达水平,分析上述指标在不同组别间的差异。结果 A组、B组患者血清RBP4水平均显著高于对照组(U值分别为-3.355、-3.600,P<0.05);A组、B组患者子宫内膜组织RBP4、PPARγ的表达水平均显著高于对照组(RBP4:U值分别为-2.695、-3.201;PPARγ:U值分别为-3.228、-3.267;P<0.05)。患者血清RBP4水平与子宫内膜组织的RBP4、PPARγ表达水平呈正相关(r值分别为0.445、0.415,P<0.05)。结论 血清RBP4水平不仅与子宫内膜增生相关,还与子宫内膜组织的生物学指标变化过程相一致。提示血清RBP4有望作为早期预测子宫内膜增生发展、恶变及预后的标志物。Objective To investigate the relationship between the levels of serum retinol binding protein 4(RBP4) and RBP4 and peroxisome proliferator-activated receptor γ(PPARγ) in endometrial tissue and endometrial hyperplasia.To analyze the relationship between the level of serum RBP4 and the expression levels of RBP4 and PPARγ in endometrial tissue.Methods A total of 48 patients with abnormal uterine bleeding who were admitted to the department of reproductive health, Shenzhen Bao′an Women′s and Children′s Hospital from July 2017 to December 2018 were selected.According to pathological classification, they were divided into the group A(n=24):endometrium with or without atypical hyperplasia(simple hyperplasia or complex hyperplasia),and the group B(n=6):endometrial atypical hyperplasia or endometrial carcinoma.The control group(n=18) was the normal endometrium group(endometrium in proliferative or secretory stage).The level of serum RBP4 and the expression levels of RBP4 and PPARγ in endometrial tissue were detected, and the differences of the above indexes among different groups were analyzed.Results The level of serum RBP4 in the group A and the group B were higher than that in the control group(U=-3.355 and-3.600,respectively, P<0.05).The expression levels of RBP4 and PPARγ in endometrial tissue in the group A and the group B were higher than those in the control group(RBP4:U=-2.695 and-3.201,respectively;PPARγ:U=-3.228 and-3.267,respectively;P<0.05).The level of serum RBP4 was correlated with the expression levels of RBP4 and PPARγ in endometrial tissue(r=0.445 and 0.415,respectively, P<0.05).Conclusion The level of serum RBP4 is not only related to endometrial hyperplasia, but also consistent with the changing process of biological indexes in endometrial tissue.It is suggested that serum RBP4 may be used as a marker for early prediction of the development, malignant transformation and prognosis of endometrial hyperplasia.
