摘　　要：李思康主任认为,肩关节周围炎病机有二,一是肩部气血阻滞,运行不畅,不通则痛;二是年老体弱,气血亏虚,筋脉得不到气血濡养,不荣则痛,日久不愈,终使肩关节屈伸不利。提出“一清二温三补”的治疗原则,“一清”即通过运用青龙摆尾针刺法调节肩关节局部的气机,疏通经络,清除局部瘀血;“二温”为对肩前(经外奇穴)、肩髃(手阳明经穴)、肩髎(手少阳经穴)三穴施以温针灸法;“三补”为针刺手四针穴,即曲池、合谷、外关、后溪。李主任重视传统复式针刺手法的运用,采用青龙摆尾针刺法,坚持以气血立论,把“一清二温三补”原则贯彻到肩关节周围炎的诊治中,取得良好疗效。Director Li Si-kang believes that there are two pathogeneses of periarthritis of the shoulder:First,the blood and qi of the shoulder are blocked and the operation is not smooth,making it blocked and painful.Second,Because of being old and weak,qi and blood deficiency,muscles and veins cannot be nourished by qi and blood,which will lead to pain and eventually unfavorable flexion and extension of shoulder joints.He has put forward three treatment principles:"combination of cleaning method and warming method and tonifying method".The clearing method refers to the use of"blue dragon wagging tail"acupuncture to adjust the local qi mechanism of the shoulder joint,dredge the meridians and clear the local blood stasis.The warming method means that warming needle moxibustion are given to three points,namely Jianqian(which is an extra point),Jianyu(LI 15,which lies in the meridian of hand yang brightness),and Jianliao(SJ 14,which lies in the meridian of hand lesser yang).The tonifying method means that acupuncture is given to the four points in the hand,including Quchi(LI 11),Hegu(LI 4),Waiguan(SJ 5)and Houxi(SI 3).Director Li attaches importance to the application of traditional compound acupuncture,adopts the"blue dragon wagging tail"needling method,adheres to the theory of qi and blood,and thoroughly implements the principle of"clearing method and warming method and tonifying method"into the diagnosis and treatment of periarthritis of shoulder,and has achieved good curative effects.
