摘  要:目的:观察通督调神灸对脊髓损伤(spinal cord injury,SCI)大鼠运动功能及炎性因子表达的影响。方法:60只雄性SD大鼠采用改良Allens法制作脊髓中度损伤模型,将造模成功的大鼠随机分为模型组和通督调神灸组,每组各30只。造模完成后,模型组大鼠仅给予预防感染等处理,通督调神灸组大鼠除预防感染外予以通督调神灸治疗,取百会、大椎、至阳、命门等穴,每穴5 min,每天1次,连续7 d。分别于术后第1天、第3天、第7天各取10只大鼠,选择Basso-Beattie-Bresnahan(BBB)评分法评估脊髓损伤后的运动功能;ELISA法检测脊髓组织中高迁移率族蛋白B1(high mobility group protein 1,HMGB1)、白细胞介素-1β(interleukin-1β,IL-1β)、IL-18、核转录因子-κB(nuclear factor kappa B,NF-κB)的蛋白表达水平。结果:术后第1天,各组大鼠双下肢均出现迟缓性瘫痪,未见明显双下肢各关节的活动;术后第3天,各组大鼠双下肢部分关节有轻度活动;术后第7天,模型组大鼠双下肢各关节能够轻度伸缩运动,但不能支撑站立,通督调神灸组大鼠双下肢各关节可见较大范围活动。术后第3天及第7天,与模型组比较,通督调神灸组大鼠的BBB评分升高,脊髓组织中HMGB1、IL-1β、IL-18、NF-κB表达水平下降;与术后第1天比较,术后第3天和术后第7天各组大鼠的BBB评分升高,脊髓组织内HMGB1、IL-1β、IL-18、NF-κB的表达水平下降;与术后第3天比较,术后第7天各组大鼠的BBB评分升高,脊髓组织内HMGB1、IL-1β、IL-18、NF-κB的表达水平下降,差异有统计学意义(P<0.01)。结论:通督调神灸能够提高SCI大鼠运动功能,抑制脊髓组织内HMGB1、IL-1β、IL-18、NF-κB的表达,改善脊髓组织内环境,减轻脊髓神经损伤,促进脊髓的神经功能恢复。Objective:To observe the influence of moxibustion for dredging the GV and regulating the mind on motor function and expressions of inflammatory factors in rats with spinal cord injury(SCI).Methods:A total of 60 male SD rats were used to make the model of moderate spinal cord injury by modified Allens method.The successfully modeled rats were randomly divided into the model group and the group of moxibustion for dredging the GV and regulating the mind,with 30 rats in each group.After the completion of modeling,the rats in the model group were only given treatment such as infection prevention.The rats in the group of moxibustion for dredging the GV and regulating the mind were additionally given moxibustion for dredging the GV and regulating the mind besides infection prevention.Baihui(DU 20),Dazhui(DU 14),Zhiyang(DU 9),Mingmen(DU 4)and other points were taken for moxibustion,each point for 5 minutes,once a day,for 7 continuous days.Then 10 rats were taken on the 1 st,3 rd and 7 th day after operation respectively,and Basso-Beattie-Bresnahan(BBB)scoring method was selected to evaluate the motor function after spinal cord injury.The protein expression levels of high mobility group protein 1(HMGB1),interleukin-1β(IL-1β),IL-18,and nuclear factor-kappa B(NF-κB)in spinal cord tissue were detected by ELISA method.Results:On the first day after operation,the rats in each group had delayed paralysis of both lower limbs,and no obvious movement of the joints of both lower limbs was observed.On the 3 rd day after operation,some joints of both lower limbs of rats in each group showed slight activity.On the 7 th day after operation,the joints of both lower limbs of rats in the model group were able to stretch slightly,but could not stand up.The joints of both lower limbs of rats in the group of moxibustion for dredging the GV and regulating the mind were able to move in a large range.On the 3 rd and 7 th day after operation,compared with the model group,BBB score of rats in the group of moxibustion for dredging the GV and re
