摘  要:目的运用基于体素的形态学(VBM)分析急性加重期慢性阻塞性肺疾病(AECOPD)患者的脑灰质体积(GMV)改变。方法前瞻性选择25例AECOPD患者(AECOPD组)及性别、年龄相匹配的20例健康受试者(对照组)为研究对象,行T_(2)-液体衰减反转恢复(FLAIR)及磁敏感加权成像(SWI)检查排除脑实质病变(脑梗死、脑炎、脑肿瘤、脑出血等),行高分辨3D-T_(1)WI检查获得脑结构数据,运用CAT_(1)2及SPM12软件分析2组脑GMV变化,FDR矫正P<0.05为差异有统计学意义。结果VBM结果显示,AECOPD组相对于对照组,双侧小脑、下丘脑、左岛叶、左扣带回、右中央旁小叶及双侧额上回GMV减少(FDR矫正P<0.05)。结论AECOPD组相较于对照组存在多个脑GMV减少,提示AECOPD患者呼吸意识发起、执行以及情绪调节的脑功能区可能存在灰质结构异常。Objective To investigate the changes of gray matter volume(GMV)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)via voxel-based morphometry(VBM).Methods A total of 25 patients with AECOPD(AECOPD group)and other 20 gender-and age-matched healthy subjects(control group)were prospectively selected.T_(2)-fluid attenuated inversion recovery(FLAIR)and susceptibility weighted imaging(SWI)were performed to exclude the cerebral parenchymal lesions(cerebral infarction,encephalitis,brain tumor,cerebral hemorrhage,etc.),and high-resolution 3D-T_(1)WI was performed to obtain brain structure data.Furthermore,all data were preprocessed and analyzed via the CAT 12 and SPM12 software.FDR correction P<0.05 was considered as statistical significance.Results VBM results showed that GMV of the bilateral cerebella,hypothalamus,left insulae,left cingulate gyrus,right paracentral lobules,and bilateral superior frontal gyrus of patients with AECOPD were significantly lower than those of the control group(FDR correction P<0.05).Conclusion Compared to control group,there are GMV reductions in multiple brain regions in patients with AECOPD.It suggestes that AECOPD patients'gray matter structures may be abnormal in the functional areas of respiratory consciousness initiation,execution,and emotion regulation.
