摘  要:目的探讨肝脏钆塞酸二钠(Gd-EOB-DTPA)增强MRI的B1场不均匀校正T_(1)mapping技术定量评估肝功能的价值。方法前瞻性选取因慢性乙型病毒性肝炎或怀疑有肝脏局灶性病变行Gd-EOB-DTPA增强MRI检查的患者69例。根据Child-Pugh评分分为3组:对照组、肝硬化Child-Pugh A级(LCA)组、肝硬化Child-Pugh B+C级(LCBC)组。根据临床资料行肝功能Child-Pugh分级,测量用于评估肝功能的3个MR参数:肝胆特异期T_(1)测量值(T_(1)post),增强后T_(1)弛豫时间减低率(ΔT_(1)),肝脏吸收指数(Khep)。采用单因素方差分析比较不同肝功能组患者间T_(1)post、ΔT_(1)、K_(hep)差异,采用受试者工作特征(ROC)曲线评估3个参数鉴别肝功能分级的效能。结果不同肝功能分组患者间MR参数均有统计学差异(P<0.05)。T_(1)post随肝功能受损程度加重而延长,而ΔT_(1)、K_(hep)随肝功能受损程度加重而减低。利用T_(1)post、ΔT_(1)、Khep鉴别诊断对照组与LCA组、对照组与LCBC组、LCA组与LCBC组的曲线下面积(AUC)分别为0.769、0.702、0.683,0.908、0.918、0.932和0.849、0.916、0.926。结论Gd-EOB-DTPA增强MRI T_(1)_mapping技术可对肝脏整体功能进行初步定量评估,对中重度肝功能不全的患者具有较高的诊断价值。Objective To explore the value of Gd-EOB-DTPA enhanced T,mapping MRI with B1 inhomogeneity correction technique in evaluating liver function.Methods Sixty-nine patients with chronic hepatitis B disease or suspected of liver focal disease who underwent Gd-EOB-DTPA enhanced MRI were prospectively selected.These patients were divided into three subgroups according to the Child-Pugh score as follows:control group,liver cirrhosis Child-Pugh class A(LCA)group,liver cirrhosis Child-Pugh class B+C(LCBC)group.Child-Pugh classification of liver function was performed according to clinical data,three MR parameters were measured to assess liver function:T_(1post),△T_(1),and K_(hep).One-way ANOVA analysis was used to compare the differences in T_(ipost),△T_(1),and K_(hep) of subgroups with different liver function.Receiver operating characteristic(ROC)curves were used to evaluate the efficacy of the three MR parameters between different subgroups.Results There were statistical differences in those MR parameters between different liver function subgroups(P<0.05).The Tipost prolonged,the △T_(1) and K_(hep) decreased with the aggravation of liver function.Area under the curve(AUC) of T_(ipost),△T_(1) and K_(hep) for distinguishing control group from LCA group were 0.769,0.702,0.683,respectively;and those for distinguishing control group from LCBC group were 0.908,0.918,0.932,respectively;and those for distinguishing LCA group from LCBC group were 0.849,0.916,0.926,respectively.Conclusion Gd-EOB-DTPA enhanced T,mapping MRI can provide quantitative information on global liver function.It shows relative high diagnostic value in patients with moderate to severe hepatic sufficiency.
