摘 要:目的探讨MRI脂肪分数图在腹部脂肪分区测量中的应用价值。方法对68例志愿者行全腹部CT平扫和腹部MRI检查。MRI脂肪分数图采用IDEAL-IQ技术获得L4/L5平面的脂肪分数图,调整不同阈值分别获取不同阈值下的内脏脂肪面积(VAT)和皮下脂肪面积(SAT)。以CT测量的VAT、SAT为标准对不同阈值测量的MRI结果进行相关性分析及Bland-Altman分析;并计算出不同阈值下测量结果的偏倚值,对偏倚值进行方差分析。结果68例志愿者的体质量指数(BMI)及腹围与VAT、SAT均有相关性,肝脏脂肪分数与VAT显著相关,而体脂率只与SAT相关。CT测量的VAT与各阈值MRI测量的VAT结果均呈高度相关性,相关系数(r)分别为0.95、0.96、0.97、0.97、0.97,P值均<0.001,但不同阈值下测量结果与CT测量的VAT偏倚值(差值)不同,其中VAT_(40)偏倚值平均值最小,为(4.03±10.80)cm^(2),四分位间距仅为10.98 cm^(2)。CT测量的VAT与40%阈值测量的VAT进行Bland-Altman一致性分析,置信区间(CI)外病例数为1/63(P<0.05)。CT测量的SAT与不同阈值下测量的SAT均呈高度相关性,r分别为0.99、0.99、0.99、0.99、0.99,P值均<0.001,其中50%阈值测量的SAT偏倚值平均值最小,为(1.11±10.24)cm^(2),四分位间距仅为15.46 cm^(2)。CT测量的SAT与50%阈值SAT进行Bland-Altman一致性分析,CI外病例数为3/68(P<0.05)。结论利用MRI脂肪分数图测量的结果与CT测量的结果呈高度相关,MRI脂肪分数图40%阈值下测量的VAT与CT测量的VAT可以相互取代,而50%阈值下测量的SAT则与CT测量的SAT可以相互取代。Objective To investigate the application value of MRI fat fraction map in abdominal fat partition measurement.Methods A total of 68 volunteers were given a plain CT scan of the whole abdomen and an MRI scan of the abdomen.The fat fraction map in the lumbar 4/5 plane was obtained using the IDEAL-IQ technology.The visceral adipose tissue(VAT)and subcutaneous adipose tissue(SAT)at different thresholds were obtained by adjusting different thresholds.Correlation analysis and Bland-Altman analysis were performed on MRI results with different thresholds based on VAT and SAT measured by CT.The bias values of measurement results with different thresholds were calculated and the analysis of variance of the bias values were analyzed.Results The body mass index(BMI)and abdominal circumference of 68 volunteers were correlated with VAT and SAT.The liver fat fraction was significantly correlated with VAT,while the body fat rate was only correlated with SAT.VAT measured by CT was highly correlated with that measured by MRI at each threshold,with r of 0.95,0.96,0.97,0.97,and 0.97,respectively,P<0.001.However,the bias values(differences)of VAT measured by different thresholds were different from that of VAT measured by CT,among which the mean bias value of VAT_(40) was at least(4.03±10.80)cm^(2),and the interquartile interval was only 10.98 cm^(2).The Bland-Altman consistency analysis between the CT-measured VAT and the 40%threshold VAT,and the number of cases outside the confidence interval(CI)was 1/63(P<0.05).SAT measured by CT was highly correlated with that measured under different thresholds,with r of 0.99,0.99,0.99,0.99,and 0.99,respectively,P<0.001.The minimum mean value of bias for SAT measured by 50%threshold was(1.11±10.24)cm^(2),and the interquartile interval was only 15.46 cm^(2).The Bland-Altman consistency analysis between the CT-measured SAT and the 50%threshold SAT,the number of cases outside the CI was 3/68(P<0.05).Conclusion The results measured by MRI fat fraction maps are highly correlated with the CT measure
