摘　　要：目的比较明胶海绵微粒和Embosphere微球经子宫动脉栓塞术(UAE)治疗子宫肌瘤(UL)的临床效果及其安全性。方法选取214例UL患者分为明胶海绵微粒治疗组(微粒组)和Embosphere微球治疗组(微球组),记录2组患者治疗前、后子宫及肌瘤体积、术后不良反应、并发症以及女性激素水平变化情况,进行比较分析。结果2组患者分别在接受治疗后3个月、6个月与治疗前比较,子宫及肌瘤体积的变化均有统计学差异(P<0.05)。然而,治疗后3个月、6个月,2个治疗组之间比较,子宫和肌瘤体积的变化无统计学差异(P>0.05)。相比微球组,微粒组术后24 h内疼痛明显,有统计学差异(P<0.05)。2组术后并发症及不良反应未见明显统计学差异(P>0.05)。结论明胶海绵微粒和Embosphere微球都可作为经UAE治疗UL的首选栓塞材料。Objective To compare the clinical effect and safety of gelatin sponge particles and Embosphere microspheres in the treatment of uterine leiomyomas(UL)by uterine artery embolization(UAE).Methods A total of 214 patients with UL were selected and divided into treatment group with gelatin sponge particles(particles group)and Embosphere microspheres(microspheres group),respectively.The uterine and myoma volume,postoperative adverse reactions,complications,and the alteration of female hormone levels were recorded before and after treatment in the two groups,and further performed a comparative analysis.Results Compared with the before treatment,the change of the uterine and myoma volume were statistically significant at the 3 and 6 months after treatment in the two groups(P<0.05).However,there were no statistically significances in the size of the uterine and myoma volume between the two treatment groups(P>0.05).Compared with the microspheres group,the pain within 24 hours was significantly obvious in particles group after treatment,and there was a statistical difference(P<0.05).There were no significant differences in the postoperative complications and the adverse reactions between the two groups(P>0.05).Conclusion Both gelatin sponge particles and Embosphere microspheres should be used as the preferred embolization materials for the treatment of UL by UAE.
