升主动脉导丝抓捕技术在Stanford B型主动脉夹层Castor单分支支架中的应用体会

摘　　要：目的:探讨升主动脉导丝抓捕技术在Castor分支支架置入治疗Stanford B型主动脉夹层中的应用效果及体会。方法:回顾性分析2021年1月至2022年3月,我院收治的行Castor分支支架置入治疗的Stanford B型主动脉夹层患者15例,所有患者近端锚定区距离左锁骨下动脉的距离均≤15 mm。记录患者手术情况、并发症情况及预后。结果:患者手术成功率为100.0%(15/15),其中急性B型夹层14例,慢性B型夹层1例,患者术后1、3个月胸主动脉真腔直径较术前扩大,假腔直径较术前缩小(P<0.05),无脑梗死并发症发生,随访过程发现分支支架闭塞1例,近端少许内漏1例,3个月复查内漏消失。结论:升主动脉导丝抓捕技术联合Castor一体式分支支架置入,治疗近端锚定区不足的Stanford B型主动脉夹层安全、有效,能够有效简化手术方式。Objective:To investigate the application effect and experience of ascending aortic guide wire capture technology in the treatment of Stanford B aortic dissection with castor branch stent implantation.Methods:15 patients with Stanford type B aortic dissection treated with castor branch stent implantation in our hospital from January 2021 to March 2022 were analyzed retrospectively.The distance between the proximal anchoring area and the left subclavian artery was≤15 mm in all patients.The operation,complications and prognosis were recorded.Results:the success rate of the operation was 100%(15/15),including 14 cases of acute type B dissection and 1 case of chronic type B dissection.The diameter of the true lumen of the thoracic aorta was larger and the diameter of the false lumen was smaller than that before operation 1 and 3 months after operation(P<0.05).There were no complications of cerebral infarction.One case of branch stent occlusion and one case of proximal leakage occurred during follow-up.Conclusions:ascending aortic guide wire capture technology combined with castor integrated branch stent implantation is safe and effective in the treatment of Stanford B aortic dissection with insufficient proximal anchorage area,and can effectively simplify the operation method.
