摘　　要：目的:探讨超声心动图评价大动脉炎患者的应用价值。方法:2002年1月至2019年6月,大动脉炎住院患者1186例,回顾性分析行心脏、大血管手术94例患者的临床资料,超声心动图结果及实验室检查,了解其心脏大血管病变特点。结果:94例患者中,男性32例(34%)和女性62例(66%),其中心脏受累74例(78.7%);以瓣膜病多见,共计51例,占68.9%(51/74),瓣膜病中,以主动脉瓣关闭不全最为常见,共计47例,占92.2%(47/51);其次为主动脉病变23例(31.1%,23/74),高血压心脏病改变20例(27%,20/74),冠心病10例(13.5%,10/74),另有扩张性心肌病表现2例(2.7%,2/74),先天性心脏病表现1例(1.4%,1/74),超声心动图未见明显异常20例(21.3%,20/94)。结论:大动脉炎患者心脏受累并不少见,尤其是年轻女性,当发现瓣膜病变特别是主动脉瓣关闭不全时需进一步检查除外大动脉炎诊断。Objective:To evaluate the value of echocardiography in patients with arteritis.Methods:From January 2002 to June 2019,1186 patients with arteritis were hospitalized.The clinical data,results of echocardiography and laboratory examination of 94 patients who underwent cardiac and great vessel surgery were retrospectively analyzed to explore the characteristics of cardiac macrovascular disease.Results:32 males(34%)and 62 females(66%)of the 94 patients were enrolled,including 74 cases(78.7%)of cardiac involvement.Valve disease was the most common in 51 cases(68.9%,51/74),and aortic insufficiency was the most common in 47 cases(92.2%,47/51).There were 23 cases of arterial disease(31.1%,23/74),20cases of hypertensive heart disease(27%,20/74),10 cases of coronary heart disease(13.5%,10/74),2 cases of dilated cardiomyopathy(2.7%,2/74),1 case of congenital heart disease(1.4%,1/74).Echocardiography showed no obvious abnormality in 20 cases(21.3%,20/94).Conclusions:Cardiac involvement is not uncommon in patients with aortitis,especially in young women.Further examination is required to exclude the diagnosis of aortitis when valvular lesions,especially aortic insufficiency is found.
