磁共振定位超声导引前列腺穿刺活检的价值分析

摘　　要：目的:探究磁共振定位超声导引实施于前列腺穿刺活检中的应用价值。方法:选取滕州市中心人民医院2021年8月—2022年5月期间接收的疑似前列腺病变患者66例,在对其进行穿刺活检前首先实施磁共振定位确定病变区域,再予以超声导引,收集所得样本组织后送病理检查室进行检查明确诊断结果,并将所得诊断结果与手术病理检查结果进行对比分析。结果:66例疑似前列腺病变患者中,手术病理检查结果显示阳性31例,其中前列腺癌4例,不典型性腺瘤样增生21例,前列腺内瘤变6例;阴性为良性前列腺增生35例。磁共振定位超声导引穿刺活检35例良性前列腺增生诊断准确,但有1例前列腺癌、2例不典型性腺瘤样增生、1例前列腺内瘤变被漏诊为良性前列腺增生。磁共振定位超声导引穿刺活检诊断灵敏度为87.10%,特异度为100.00%,准确率为93.94%,具有高度一致性(Kappa=0.877)。磁共振定位超声导引穿刺活检方式下共计有6例患者出现相关并发症,主要有:血便2例、排尿困难1例、发热3例,但经对症处理后,均有明显好转或消失。结论:在前列腺穿刺活检中应用磁共振定位超声导引具有较为显著的临床应用价值,诊断效能高、并发症少、操作较为简单便捷,对疾病的诊疗具有重要参考作用,可加以积极推广运用。Objective To explore the application value of magnetic resonance positioning ultrasound guidance in prostate biopsy.Methods A total of 66 patients with suspected prostate lesions who were admitted to Tengzhou Central People's Hospital from August 2021 to May 2022 were selected.MRI localization was performed to determine the lesion area before needle biopsy,and then ultrasound guidance was performed.The obtained samples were collected and sent to the pathological examination room for examination to confirm the diagnosis results,and the obtained diagnosis results were compared with the surgical pathological examination results.Results Among 66 patients with suspected prostate lesions,31 cases were positive by surgical pathological examination,including 4 cases of prostate cancer,21 cases of atypical adenomatous hyperplasia,and 6 cases of intraprostatic neoplasia.35 cases were negative for benign prostatic hyperplasia.35 cases of benign prostatic hyperplasia were accurately diagnosed by ultrasoundguided biopsy.However,1 case of prostate cancer,2 cases of atypical adenomatous hyperplasia and 1 case of intraprostatic neoplasia were missed as benign prostatic hyperplasia.The sensitivity,specificity and accuracy of MRI localization ultrasound-guided puncture biopsy were 87.10%,100.00%and 93.94%,respectively,with high consistency(Kappa=0.877).A total of 6 patients developed related complications under ultrasound-guided needle biopsy with magnetic resonance localization,including 2 cases of bloody stool,1 case of dysuria,and 3 cases of fever,however,after symptomatic treatment,there were obvious improvement or disappearance.Conclusion The application of magnetic resonance positioning ultrasound guidance in prostate biopsy has significant clinical application value,with high diagnostic efficiency,fewer complications,and simple and convenient operation.
