沙库巴曲缬沙坦治疗老年射血分数降低的心力衰竭患者的临床疗效

摘　　要：目的观察沙库巴曲缬沙坦治疗老年射血分数降低的心力衰竭(HFpEF)患者的临床疗效。方法选取2020年3月—2021年9月绵阳市中心医院收治的102例老年HFpEF患者作为研究对象,采用随机数字表法分为对照组51例和观察组51例。对照组接受坎地沙坦酯治疗,观察组采用沙库巴曲缬沙坦治疗。2组患者均连续治疗6个月。比较2组治疗前及治疗6个月心功能指标[左心室圆周应变(GCS)、径向应变(GRS)、整体纵向应变(GLS)、左心室射血分数(LVEF)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)]、神经内分泌激素[醛固酮(ADS)、血管紧张素Ⅱ(AngⅡ)、去甲肾上腺素(NE)]水平,并观察患者不良事件发生情况。结果2组治疗前GCS、GRS、GLS、LVEF、LVEDV、LVESV比较,差异无统计学意义(P>0.05);治疗6个月后,2组GCS、GLS低于治疗前,GRS、LVEF高于治疗前,LVEDV、LVESV小于治疗前,且观察组GLS低于对照组,LVEF高于对照组(P<0.01)。治疗6个月后,2组血浆ADS、AngⅡ、NE水平低于治疗前,且观察组低于对照组(P<0.01)。2组不良事件总发生率比较,差异无统计学意义(P=1.000)。结论沙库巴曲缬沙坦可有效改善老年HFpEF患者的心功能,抑制神经内分泌激素释放,进而减轻心脏负荷,促进心功能恢复,且安全性较高。Objective To investigate the therapeutic effect of sabatrix valsartan in the treatment of elderly patients with heart failure with reduced ejection fraction(HFpEF).Methods A total of 102 elderly patients with HFpEF treated in Mianyang Central Hospital from March 2020 to September 2021 were selected,and they were divided into control group(51 cases)and observation group(51 cases)according to random number table method.Patients in the control group was treated with candesartan,patients in the observation group was treated with sabatrix valsartan,both groups were continuously treated for 6 months.Cardiac function indexes(GCS,GRS,GLS,LVEF,LVEDV,LVESV),neuroendocrine hormone levels(ADS,AngⅡ,NE)before treatment and at 6 months after treatment,incidence of adverse events were compared between the two groups.Results There was no significant difference of GCS,GRS,GLS,LVEF,LVEDV,LVESV before treatment and GCS,GRS,LVEDV,LVESV at 6 months after treatment between the two groups(P>0.05);at 6 months after treatment,GCS,GLS in the two groups were lower than those before treatment,GRS,LVEF were higher than those before treatment,LVEDV,LVESV were less than those before treatment,and GLS in observation group was lower than that in control group,LVEF was higher than that in control group(P<0.01).At 6 months after treatment,plasma levels of ADS,AngⅡ,NE in the two groups were lower than those before treatment,and those in observation group were lower than control group(P<0.01).There was no significant difference of incidence of adverse events between the two groups(P=1.000).Conclusion Sabatrix valsartan can effectively improve cardiac function of elderly patients with HFpEF,and can inhibition of neuroendocrine hormone release,thereby reduce the load on the heart,promote the recovery of cardiac function,and with high safety.
