摘　　要：目的:研究骨科药师根据国家政策要求、患者特点、医生困惑等方面协助参与药物治疗管理的策略。方法:利用SWOT分析法分析骨科临床药师职业现状,剖析与骨科临床药师职业密切相关的各种因素。结果:临床药师在DRGs(疾病诊断相关分类)付费模式下协助管理者实行药品精细化管理,改进后,围术期预防用药日均次药费减少约150元,脊柱、创伤、关节、运动医学均次药费环比各降低55.27%、41.42%、64.40%、51.09%,不合理医嘱率由40.48%下降至2022年2月份的3.57%;针对慢病患者制定同质化用药教育模板,建立用药教育标准化制度,加强护理用药知识宣教,形成标准化流程,帮助患者提升用药依从性,患者满意率由改进前91.3%提升至93.4%。以抗感染为切入点融入临床,协助医生制定有效的用药方案;协助临床制定围术期止痛方案等,药师会诊医生接纳率高达95.7%,根据药师方案用药的有效率94.7%。结论:骨科药师发挥优势,抓住机遇,克服劣势,以骨科问题为导向,协助药品精细化管控以实现临床合理用药,多维度融入诊疗团队,提升骨科药师药学服务能力。Objective:To study strategies for orthopedic pharmacists to assist in drug treatment management according to national policy requirements,patient characteristics,doctors'confusion,etc.Methods:Through the analysis of present situation of clinical pharmacist profession,application of SWOT analysis detailed factors that were closely related to orthopedic clinical pharmacist professional.Results:Clinical pharmacists assisted managers in the DRGs payment model,the average daily drug cost of perioperative preventive medication was reduced by about 150 yuan,and the spine,trauma,joint,sports medicine average drug cost each decreased by 55.27%,41.42%,64.40%,51.09%,and the unreasonable medical order rate decreased by 40.48%to 3.57%in February 2022;for patients with chronic diseases,a homogeneous drug use education template was formulated,a standardized system of drug use education was established,nursing drug use knowledge education was strengthened,and standardized processes were formed to help patients improve drug compliance.The satisfaction rate of patients was increased from 91.3%to 93.4%.Take anti-infection as the entry point into clinical integration,assist doctors to formulate effective medication plan;assist the clinic to formulate perioperative analgesic plan,et al.The acceptance rate of pharmacist consultant was up to 95.7%,and the effective rate of medication according to the pharmacist plan was 94.7%.Conclusion:Orthopedic pharmacists take advantage,seize the opportunity,overcome the disadvantage,with orthopaedic problem oriented,to assist the drug refinement control to realize clinical rational drug use,multidimensional into the diagnosis and treatment team,improve orthopaedic pharmacists pharmaceutical service ability.
