摘　　要：目的:探究他克莫司软膏联合盐酸依匹斯汀胶囊治疗轻中度特应性皮炎(AD)临床疗效及炎性因子水平的影响。方法:选择80例江西省赣州市皮肤病医院2019年1月至2021年7月收治的轻中度AD患者,采用随机数字表法分为对照组及观察组,各40例。所有患者均先于皮损处涂抹薇诺娜舒敏保湿特护霜,对照组单用丁酸氢化可的松乳膏,观察组采用他克莫司软膏联合盐酸依匹斯汀胶囊治疗,持续用药3周,之后随访3个月。对比两组治疗效果。结果:相比于对照组(77.50%),观察组治疗总有效率(95.00%)较高(P<0.05);观察组治疗后IL-13水平为(27.36±2.26)ng/L、IL-17水平为(24.52±4.26)ng/L,均低于对照组的(31.72±2.51)ng/L、(30.44±3.25)ng/L,IFN-γ水平为(14.72±2.27)ng/L高于对照组的(13.18±2.13)ng/L,均差异有统计学意义(P<0.05);相比于对照组(25.00%),观察组复发率(7.50%)较低(P<0.05);观察组不良反应发生率为2.50%,对照组为15.00%,两组对比差异无统计学意义(P>0.05)。结论:他克莫司软膏联合盐酸依匹斯汀胶囊治疗轻中度AD效果确切,可有效减轻炎症反应,降低复发率,且安全性高。Objective:To explore the effect of tacrolimus ointment combined with epinastine hydrochloride capsules on the clinical efficacy and the level of inflammatory factors in the treatment of mild to moderate atopic dermatitis(AD).Methods:A total of 80 patients with mild to moderate AD who were admitted to Ganzhou Dermatosis Hospital from January 2019 to July 2021 were selected and divided into the control group and the observation group by random number table method,with 40 cases in each group.All patients were treated with winona shumin moisturizing special cream before the skin lesions,the control group was treated with hydrocortisone butyrate cream alone,and the observation group was treated with tacrolimus ointment combined with epinastine hydrochloride capsules,and the medication was continued 3 weeks,followed by a 3-month follow-up.The treatment effects of the two groups were compared.Results:Compared with the control group(77.50%),the total effective rate(95.00%)of the observation group was higher,with statistical difference(P<0.05).The IL-13 level of the observation group after treatment was(27.36±2.26)ng/L and IL-17 were(24.52±4.26)ng/L,which were lower than(31.72±2.51)ng/L and(30.44±3.25)ng/L of the control group,and the level of IFN-γwas(14.72±2.27)ng/L was significantly higher than that of(13.18±2.13)ng/L in the control group(P<0.05).Compared with the control group(25.00%),the recurrence rate of the observation group(7.50%)was lower,with statistical difference(P<0.05).The incidence of adverse reactions was 2.50%in the observation group and 15.00%in the control group,and there was no statistical difference between the two groups(P>0.05).Conclusion:Tacrolimus ointment combined with epinastine hydrochloride capsules is effective in the treatment of mild to moderate AD,can effectively reduce the inflammatory response,reduce the recurrence rate,and has high safety.
