隔代照料与我国老年人医疗服务利用的关系

摘　　要：目的:分析老年人提供隔代照料及不同照料强度对其医疗服务利用的影响,并比较不同年龄、性别和城乡差异,以期为老年人健康促进政策制定提供循证决策依据。方法:基于2018年CHARLS数据,以6641名60岁及以上的老年人为研究对象,采用随机截距logit及OLS模型进行实证分析。结果:提供隔代照料的老年人住院发生率及住院次数相对较低(OR=0.764,β=-0.099),体检发生率却相对较高(OR=1.142);相较于不提供隔代照料的老年人,提供高强度照料的老年人住院服务利用相对较少(OR=0.764,β=-0.103);异质性检验的结果表明隔代照料对老年人医疗服务利用的影响存在年龄、性别和城乡差异。结论:隔代照料及照料强度会显著影响老年人的就医行为,应完善对隔代照料老年群体的帮扶政策,合理释放其就医需求。Objective:To analyze the impact of providing intergenerational care and different care intensity on the utilization of medical services,and compare the differences between different ages,genders and urban and rural areas,so as to provide evidence-based decision-making basis for the formulation of health promotion policies for the elderly.Methods:6641 elderly people aged 60 and older were selected as the research objects based on CHARLS data in 2018.The random intercept logit and OLS models were used to explore the impact of whether the elderly provide intergenerational care and different care intensity on medical service utilization.Results:The incidence and times of inpatient for the elderly who cared grandchildren were relatively lower(OR=0.764,β=-0.099),but the incidence of physical examination was reverse(OR=1.142).Compared with the elderly who did not provide intergenerational care,the elderly who provide high-intensity care made relatively less use of hospitalization services(OR=0.764,β=-0.103).The results of heterogeneity test indicated that there were age,gender and urban-rural differences in the impact of intergenerational care on the utilization of medical services for the elderly.Conclusion:Intergenerational care and care intensity will significantly affect the medical behavior of the elderly,so it is widely to strengthen the assistance to the elderly group of intergenerational care,further release their medical demands.
