摘 要:目的分析颈部彩色多普勒血流显像(CDFI)与经颅彩色多普勒超声(TCCS)联合应用诊断椎动脉狭窄的临床效果。方法186例椎动脉狭窄患者,均接受颈部CDFI、TCCS检查,并在7 d内接受数字减影血管造影(DSA)检查。以DSA诊断结果为金标准,分析不同检查方法对椎动脉狭窄的诊断结果。结果CDFI检出椎动脉椎间段重度狭窄9例,DSA检出椎动脉椎间段重度狭窄9例,CDFI检测椎动脉椎间段重度狭窄的符合率为100.00%(9/9)。CDFI检出椎动脉开口位置重度狭窄153例,DSA检出椎动脉开口位置重度狭窄152例,CDFI检测椎动脉开口位置重度狭窄的符合率为99.35%(152/153)。TCCS检出椎动脉颅内段狭窄24例,DSA检出椎动脉颅内段狭窄25例,TCCS检测椎动脉颅内段狭窄符合率为96.00%(24/25)。以DSA诊断结果为金标准,TCCS联合CDFI诊断椎动脉狭窄的符合率为99.46%(185/186)。结论CDFI与TCCS联合应用诊断椎动脉狭窄能取得满意效果,该法有着较高的诊断符合率,其可被视为筛查病理性椎动脉狭窄的有效方式,值得进一步在临床内推广和应用。Objective To analyze the clinical effect of cervical color Doppler flow imaging(CDFI)combined with transcranial color code sonography(TCCS)in the diagnosis of vertebral artery stenosis.Methods All 186 patients with vertebral artery stenosis underwent cervical CDFI and TCCS and digital subtraction angiography(DSA)within 7 d.The diagnostic results of DSA were used as the gold standard to analyze the diagnostic results of different examination methods for vertebral artery stenosis.Results CDFI detected 9 cases of severe stenosis of the intervertebral segment of the vertebral artery,and DSA detected 9 cases of severe stenosis of the intervertebral segment of the vertebral artery,and the coincidence rate of severe stenosis of the intervertebral segment of the vertebral artery by CDFI was 100.00%(9/9).CDFI detected 153 cases of severe stenosis in the opening position of the vertebral artery,and DSA detected 152 cases of severe stenosis in the opening position of the vertebral artery,and the coincidence rate for detecting severe stenosis in the opening position of the vertebral artery by CDFI was 99.35%(152/153).TCCS detected 24 cases of intracranial segment stenosis of the vertebral artery,and DSA detected 25 cases of intracranial segment stenosis of the vertebral artery,and the coincidence rate of intracranial segment stenosis of the vertebral artery by TCCS was 96.00%(24/25).Taking the diagnostic results of DSA as the gold standard,the coincidence rate of TCCS combined with CDFI in the diagnosis of vertebral artery stenosis was 99.46%(185/186).Conclusion The combination of CDFI and TCCS can achieve satisfactory results in the diagnosis of vertebral artery stenosis,and the method has a high diagnostic coincidence rate,which can be regarded as an effective way to screen for pathological vertebral artery stenosis and is worth further promotion and application in clinical practice.
