经阴道B超对早期异位妊娠的临床诊断价值探究

摘　　要：目的探讨经阴道B超对早期异位妊娠的临床诊断价值。方法58例早期异位妊娠产妇,均行经阴道B超和经腹部B超检查,比较两种检查方式图像特征及对妊娠类型的诊断准确率。结果经腹部B超检出3例(5.17%)假孕囊,33例(56.90%)盆腔积液,11例(18.97%)卵黄囊、胎芽、心脉管搏动;经阴道B超检出5例(8.62%)假孕囊,44例(75.86%)盆腔积液,21例(36.21%)卵黄囊、胎芽、心脉管搏动;经阴道B超对盆腔积液和卵黄囊、胎芽、心脉管搏动的检出率均高于经腹部B超,差异具有统计学意义(P<0.05);两种检查方式对假孕囊的检出率比较差异无统计学意义(P>0.05)。经阴道B超检查的诊断总准确率96.55%明显高于经腹部B超的60.34%,差异具有统计学意义(P<0.05)。结论B超影像应用于早期异位妊娠诊断能有效提高诊断准确率,特别是经阴道B超诊断符合率较高,值得推广。Objective To discuss the clinical diagnostic value of transvaginal B-mode ultrasound for early ectopic pregnancy.Methods In 58 women with early ectopic pregnancy,transvaginal B-mode ultrasound and transabdominal ultrasound were performed to compare the image characteristics and diagnostic accuracy of the two examination modalities for the type of pregnancy.Results Transabdominal ultrasound detected 3 cases(5.17%)of pseudogestational sac,33 cases(56.90%)of pelvic effusion,11 cases(18.97%)of yolk sac,fetal bud,and cardiac pulse;transvaginal ultrasound detected 5 cases(8.62%)of pseudogestational sac,44 cases(75.86%)of pelvic effusion,and 21 cases(36.21%)of yolk sac,fetal bud,and cardiac pulse.The detection rate of pelvic effusion,yolk sac,fetal bud,and cardiac vascular pulse by transvaginal B-mode ultrasound was higher than that of transabdominal ultrasound,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the detection rate of pseudogestational sac between the two inspection methods(P>0.05).The overall diagnostic accuracy rate of transvaginal B-mode ultrasound was 96.55%,which was significantly higher than 60.34%of transabdominal ultrasound,and the difference was statistically significant(P<0.05).Conclusion The application of B-mode ultrasound imaging in the diagnosis of early ectopic pregnancy can effectively improve the diagnostic accuracy,and the diagnostic coincidence rate of transvaginal ultrasound is high,which is worthy of promotion.
