摘　　要：目的分析对精子采取两种不同优化处理方法对体外受精-胚胎移植结局的影响。方法选取我院2019年1月至2021年12月辅助生殖治疗IVF标准患者中的138周期的患者为观察组,采取上游法+梯度合并上游法处理精子,334周期的患者为对照组,采取上游法对精子进行处理,对比两组胚胎妊娠结局。结果观察组正常受精率65.73%、多PN率5.17%、正常卵裂率98.08%、可用胚胎囊胚形成率74.63%、可用囊胚形成率50.00%、着床率51.75%、妊娠率68.14%、流产率9.57%、持续妊娠率61.59%,对照组正常受精率66.02%、多PN率6.20%、正常卵裂率97.99%、可用胚胎囊胚形成率75.28%、可用囊胚形成率48.73%、着床率48.73%、妊娠率64.07%、流产率11.21%、持续妊娠率56.88%,两组上述指标水平相近,组间对比差异无统计学意义(P>0.05);观察组优胚率45.85%高于对照组40.42%,组间对比差异有统计学意义(P<0.05)。结论上游法同梯度合并上游法相比可以有效提升优胚率,价值较高。Objective To analyze the effects of two different methods of sperm optimization on the outcome of in vitro fertilization and embryo transfer.Methods From January 2019 to December 2021,the patients with 138 cycles of IVF standard in our hospital were selected as the observation group,and the patients with 334 cycles were treated with upstream method+gradient combined with upstream method,while the patients with 334 cycles were taken as the control group.The results of embryo pregnancy were compared between the two groups.Results In the observation group,the normal fertilization rate was 65.73%,the multiple PN rate was 5.17%,the normal cleavage rate was 98.08%,the usable embryo blastocyst formation rate was 74.63%,the usable blastocyst formation rate was 50.00%,the implantation rate was 51.75%,the pregnancy rate was 68.14%,the abortion rate was 9.57%,and the continuous pregnancy rate was 61.59%.In the control group,the normal fertilization rate,multiple PN rate,normal cleavage rate,usable blastocyst formation rate,usable blastocyst formation rate,implantation rate,pregnancy rate,abortion rate and sustained pregnancy rate were 66.02%,6.20%,97.99%,75.28%,48.73%,48.73%,64.07%,11.21%and 56.88%,respectively(P>0.05).The above indexes of the two groups were similar.The excellent embryo rate of the observation group(45.85%)was higher than that of the control group(40.42%),with a comparison between the two groups(P<0.05).Conclusion Compared with the gradient combined with the upstream method,the upstream method can effectively improve the excellent embryo rate and has a higher value.
