促炎因子、生殖内分泌激素、脂肪因子对多囊卵巢综合征发病的影响及其交互作用

摘　　要：目的探讨促炎因子、生殖内分泌激素、脂肪因子与多囊卵巢综合征(PCOS)发病的相关性及其交互作用。方法将2020年1月-2022年1月就诊的PCOS 100例作为PCOS组,选择同期体检的健康女性100例作为健康组。比较2组体质量指数(BMI)、空腹血糖(FBG)、空腹胰岛素(INS)、生殖内分泌激素[促卵泡激素(FSH)、黄体生成激素(LH)、总睾酮(T)、抗苗勒管激素(AMH)]、脂肪因子[脂联素(ADPN)、瘦素(LEP)、视黄醇结合蛋白(RBP)]、促炎因子[超敏C反应蛋白(hs-CRP)、淀粉样蛋白A(SAA)、kruppel样因子7(KLF7)、糖类抗原125(CA125)、人趋化因子(FK)、单核细胞趋化蛋白-1(MCP-1)]水平,并分析与PCOS的相关性及其交互作用。结果POCS组BMI、FSH、LH、T、AMH、ADPN、RBP、hs-CRP、KLF7、CA125、MCP-1、INS、FBG、稳态模型-胰岛素抵抗指数(HOMA-IR)与健康组比较差异有统计学意义(P<0.05,P<0.01)。BMI、KLF7、T、LH、ADPN、hs-CRP、RBP、HOMA-IR是PCOS发病的影响因素(P<0.05,P<0.01)。BMI与脂肪因子、促炎因子及HOMA-IR均有交互作用,T与促炎因子(KLF7、ADPN、hs-CRP)及HOMA-IR有交互作用,LH与促炎因子(KLF7、ADPN、hs-CRP)及HOMA-IR有交互作用(P<0.05,P<0.01)。结论BMI、KLF7、T、LH、ADPN、hs-CRP、RBP、HOMA-IR是PCOS发病的影响因素,以上因子之间的交互作用增加了PCOS的发病风险。Objective To investigate the correlation and interaction between proinflammatory factors,reproductive endocrine hormones,adipose factors and polycystic ovary syndrome(PCOS).Methods A total of 100 PCOS patients treated from January 2020 to January 2022 were selected as the PCOS group,and 100 healthy women undergoing physical examination during the same period were selected as the healthy group.Body mass index(BMI),fasting blood glucose(FBG),fasting insulin(INS),reproductive endocrine hormones[follicle stimulating hormone(FSH),luteinizing hormone(LH),total testosterone(T),anti-Mullerian hormone(AMH)],adipokines[adiponectin(ADPN),leptin(LEP),retinol binding protein(RBP)],proinflammatory factors[hypersensitive C reactive protein(hs-CRP),serum amyloid A(SAA),kruppel like factor 7(KLF7),carbohydrate antigen 125(CA125),human chemokine(FK),monocyte chemoattractant protein 1(MCP1)]levels were compared between the two groups,and their correlation and interaction with PCOS were analyzed.Results There were significant differences in BMI,FSH,LH,T,AMH,ADPN,RBP,hs-CRP,KLF7,CA125,MCP-1,INS,FBG,homeostasis model assessment of insulin resistance(HOMA-IR)between POCS group and healthy group(P<0.05,P<0.01).BMI,KLF7,T,LH,ADPN,hs-CRP,RBP,and HOMA-IR were the influencing factors of PCOS(P<0.05,P<0.01).BMI interacted with adipose factor,proinflammatory factor and HOMA-IR,T interacted with proinflammatory factor(KLF7,ADPN,hs-CRP)and HOMA-IR,and LH interacted with proinflammatory factor(KLF7,ADPN,hs CRP)and HOMA-IR(P<0.05,P<0.01).Conclusion BMI,KLF7,T,LH,ADPN,hs-CRP,RBP,and HOMA-IR are the influencing factors of PCOS.The interaction between the above factors increases the risk of PCOS.
