天津市老年人医养照护服务需求现状调查及影响因素分析

摘　　要：目的:调查天津市老年人医养照护服务需求现状并分析其影响因素,为构建需求导向化的长期照护体系提供依据。方法:采用方便抽样法,选取天津市4713名60岁及以上的老年人进行调查,分析其需求水平的影响因素并调查其养老意愿。结果:老年人对健康监测、心理与社会支持、保护与安全方面的需求水平较高。多因素分析结果显示,年龄、配偶情况、文化程度、居住方式、使用手机的情况、过去一年因病住院次数及Barthel指数评定量表总分是老年人医养照护服务需求水平的影响因素(P均<0.05)。结论:目前天津市老年人存在医养照护服务需求未被充分满足的情况,高龄、无配偶、非初中学历、孤寡独居或居住在养老机构、未使用手机、过去一年里因病住院次数多、失能或半失能的老年人照护需求高,建议加大其长期照护服务供给力度。Objective:To describe the current characteristics of the care demand for health care services in elderly in Tianjin,and analyze the potential influencing factors,therefore provide references for the construction of a demand-oriented long-term care system in Tianjin.Methods:By using the convenient sampling method,a total of 4713 people who were 60 years of age or older in Tianjin were selected for the survey to analyze the influencing factors of their demand level and to investigate their willingness to provide for the elderly.Results:The elderly have a high demand for health monitoring,psychological and social support,protection and safety.The multivariate analysis showed that age,level of education,the number of hospitalizations due to illness,self-care ability,possession of spouse,use of mobile phones and model of living were influencing factors of demand level of medical care services for the elderly(all P<0.05).Conclusion:At present,the demand for medical care services for the elderly in Tianjin has not been fully met.The elderly who are very elderly,have no spouse,live alone or in an elderly care institution,do not use mobile phones,have been hospitalized for many times due to illness in the past year,and are disabled or semi disabled,whose the highest degree is not junior high school may have a high demand for care.It is recommended to increase the supply of long-term care services.
