摘 要:目的:探讨外周血细胞因子表达水平对孕妇胎膜早破(PROM)及新生儿围产期感染的影响。方法:选取2019年7月-2021年8月湛江市第四人民医院产科收治的50例PROM并成功分娩的足月产妇作为研究组,选取同期足月正常分娩的50例产妇作为对照组。在分娩前、分娩后24 h内,抽取产妇及新生儿的空腹外周静脉血,检测白细胞介素2(IL-2)、白细胞介素6(IL-6)、白细胞介素10(IL-10)、可溶性白细胞分化抗原14(SCD14)及干扰素γ(IFN-γ)水平,并进行组间比较。结果:分娩后,研究组IL-2、IL-6、IL-10、SCD14及IFN-γ均低于分娩前,差异有统计学意义(P<0.05);研究组分娩前后IL-2、IL-6、IL-10、SCD14及IFN-γ水平均高于对照组,差异有统计学意义(P<0.05)。研究组新生儿感染7例,感染率为14%,对照组无新生儿感染。两组新生儿IL-2、IL-6水平比较,差异无统计学意义(P>0.05);研究组新生儿SCD14、IFN-γ水平高于对照组,IL-10水平低于对照组,差异有统计学意义(P<0.05)。结论:PROM孕妇和感染新生儿的外周血炎性细胞因子表达水平显著发生变化,进行外周血细胞因子检测有利于早期预测PROM和新生儿围产期感染。Objective:To investigate the effect of peripheral blood cytokine expression level on premature rupture of membranes(PROM)in pregnant women and perinatal infection in newborns.Methods:Fifty full-term mothers with PROM and successful delivery admitted to the obstetrics department of Zhanjiang Fourth People's Hospital from July 2019 to August 2021 were selected as the study group.Fifty cases of full-term normal deliveries during the same period were selected as the control group.Before delivery and 24 h after delivery,fasting peripheral venous blood was drawn from the mothers and newborns,and the levels of interleukin 2(IL-2),interleukin 6(IL-6),interleukin 10(IL-10),soluble leukocyte differentiation antigen 14(SCD14)and interferon gamma(IFN-γ)were measured and compared between groups.Results:After delivery,IL-2,IL-6,IL-10,SCD14 and IFN-γwere lower in the study group than before delivery,and the difference was statistically significant(P<0.05);the levels of IL-2,IL-6,IL-10,SCD14 and IFN-γwere higher in the study group than in the control group before and after delivery,and the difference was statistically significant(P<0.05).There were 7 neonatal infections in the study group,with an infection rate of 14%,and no neonatal infections in the control group.There was no statistically significant difference between the levels of IL-2 and IL-6 in the neonates of the two groups(P>0.05);the levels of SCD14 and IFN-γin the neonates of the study group were higher than those in the control group,and the levels of IL-10 were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:Peripheral blood inflammatory cytokine expression levels were significantly altered in pregnant women with PROM and infected newborns.Performing peripheral blood cytokine assays is beneficial for early prediction of PROM and neonatal perinatal infection.
