摘 要:目的:分析FRAX系统[不含骨密度(BMD)模型]、BMD及骨转换指标对绝经后女性骨质疏松性骨折的预测价值。方法:选取2017年1月-2018年12月于上海市徐汇区枫林街道社区卫生服务中心骨质疏松专病门诊接受双能X线吸收法BMD检查、骨转换指标检测的747例绝经后女性作为研究对象,根据是否发生骨质疏松性骨折分为研究组(骨折)与对照组(未骨折),研究组373例,对照组374例。比较两组一般资料、FRAX评分、BMD水平、骨转换指标,Logistic回归分析绝经后女性发生骨质疏松性骨折的影响因素。结果:研究组体重、FRAX评分、腰椎BMD、股骨颈BMD、股骨BMD、25-羟基维生素D水平均高于对照组,Ⅰ型胶原羧基末端肽(β-CTX)水平高于对照组,差异有统计学意义(P<0.05)。Logistic回归分析结果显示,25-羟基维生素D、体重、股骨BMD、β-CTX水平是绝经后女性发生骨质疏松性骨折的影响因素。受试者分析曲线结果显示,FRAX及β-CTX对骨折的发生具有预测价值。结论:FRAX评分及β-CTX对骨质疏松性骨折发生具有预测价值。Objective:To analyze the predictive value of the FRAX system[without bone mineral density(BMD)model],BMD and bone turnover index for postmenopausal women with osteoporotic fractures.Methods:747 postmenopausal women who underwent BMD examination by dual-energy X-ray absorptiometry and bone turnover index testing at the osteoporosis-specific clinic of Fenglin Street Community Health Center in Xuhui District,Shanghai,from January 2017 to December 2018 were selected as study subjects.According to whether osteoporotic fracture occurred or not,they were divided into study group(fracture)and control group(no fracture),373 cases in the study group and 374 cases in the control group.The general data,FRAX score,BMD level,and bone turnover indexes of the two groups were compared,and the factors influencing the occurrence of osteoporotic fractures in postmenopausal women were analyzed by Logistic regression.Results:Body weight,FRAX score,lumbar spine BMD,femoral neck BMD,femoral BMD,25-hydroxyvitamin D levels were higher in the study group than in the control group,and type I collagen carboxy-terminal peptide(β-CTX)levels were higher than in the control group,and the differences were statistically significant(P<0.05).Logistic regression analysis showed that 25-hydroxyvitamin D,body weight,femoral BMD,β-CTX levels were influential factors in the development of osteoporotic fractures in postmenopausal women.The results of subject analysis curves showed that FRAX andβ-CTX had predictive value for the occurrence of fractures.Conclusion:FRAX score andβ-CTX have predictive value for the occurrence of osteoporotic fractures.
