摘　　要：以慢性病创新护理模式(ICCCF)为基础,进一步整合社区精神卫生防治资源,实现专科医院-社区-家庭精神卫生综合团队延续性护理服务,为精神障碍患者提供积极有效的治疗、护理和康复,更好地巩固治疗效果,减少疾病复发,提高其社会适应能力和生活质量。通过1年半的探索和实践,每半年为一个服务周期,总结可行性经验:回到社区的严重精神障碍患者在ICCCF团队服务下,社区精神卫生服务不再限于管理,而是依据患者及家庭的需求提供服务。促进社区化精神卫生服务,护理工作从专科医院向社区转移,使患者接受及时而有针对性、连续性的程式化康复技能训练,从而降低复发率,减轻残疾程度,促进患者早日回归社会。Based on innovative care for chronic conditions framework(ICCCF),the community mental health prevention and treatment resources are integrated,and the comprehensive team of the specialized hospital-community-family mental health offer the continuous nursing services,and the effective treatment,nursing and rehabilitation could be provided for the patients with mental disorders,which would better consolidate the treatment effect,reduce disease recurrence,improve social adaptability and quality of life.After the exploration and practice over 1.5 years,every six months is taken as a service cycle,the feasible experience is summarized:the patients with severe mental disorders who return to the community are served by the team with ICCCF.The community mental health services are not limited to the management,services based on the needs of patients and families are also provided.It promotes community-based mental health services and transformation of nursing work from specialized hospitals to communities,which help patients get timely,targeted and continuous programmatic rehabilitation skills training,thereby reducing the recurrence rate and the degree of disability,and promoting the early return of patients to society.
