摘　　要：目的 探讨负压封闭引流(VSD)对开放性骨折术后慢性难愈性创面患者创面修复速率及感染率的影响.方法 选取2018年5月-2021年5月华泰奎阁医院收治的70例开放性骨折术后出现慢性难愈性创面患者,采用简单随机分组方法将患者分为观察组和对照组,每组各35例.对照组患者采用常规处理,观察组患者采用VSD治疗,随访3个月,记录两组患者临床疗效,观察两组患者创面修复速率及创面愈合质量.于治疗前、治疗后3月,抽取患者的静脉血检测患者的炎性指标[白细胞介素-6(IL-6)、C-反应蛋白(CPR)、红细胞沉降率(ESR)以及降钙原素(PCT)]水平.观察两组患者不良反应发生情况.结果 观察组总有效率为94.29%,明显高于对照组的77.14%(χ^(2)=4.200,P=0.040);观察组创面愈合时间明显短于对照组,且创面愈合质量优于对照组(P<0.05);治疗3月后,两组患者的IL-6、CRP、ESR、PCT等炎症指标水平均有下降(P<0.05),其中观察组更为显著(P<0.05).观察组的创面感染发生率为5.71%,明显低于对照组的22.86%(P<0.05).结论 VSD可有效促进开放性骨折术后发生慢性难愈性创面患者的创面愈合,可缩短其创面愈合时间,提高创面愈合质量,降低创面感染几率.Objective To investigate the effect of negative pressure sealed drainage(VSD)on wound repair rate and infection rate of patients with chronic refractory wounds after open fracture.Methods 70 patients with chronic refractory wounds after open fractures treated in our hospital from May 2018 to May 2021 were selected and randomly divided into observation group and control group.with 35 cases in each group.The control group was treated with routine treatment,and the ohservation group was treated with VSD.The clinical efficacy of the two groups were recorded for 3 months,and the wound repair rate and wound healing quality of the two groups were observed.Before and 3 months after treatment,venous blood was collected from patients to measure the levels of inflammatory markers [interleukin-6(IL-6),C-reactive protein(CPR),erythrocyte sedimentation rate(ESR),and calcitonin(PCT)].The incidence of adverse reactions in 2 groups was observed.Results The total effective rate of the observation group was 94.29%.significantly higher than that of the control group(77.14%)(χ^(2)=4.200,P=0.040).The wound healing time in the observation group was significantly shorter than that in the control group.and the wound healing quality was better than that in the control group(P<0.05).After 3 months of treatment.the levels of IL-6.CRP,ESR.PCT and other inflammatory indicators in 2 groups were decreased(P<0.05),and the observation group was more significant(P<0.05).The incidence of wound infection in the ohservation group was 5.71%,which was significantly lower than that in the control group(22.86%.P<0.05).Conc l us ion VSD can effectively promote the wound healing of patients with chronic refractory wounds afier open fracture surgery.shorten the time of wound healing.improve the quality of wound healing.and reduce the incidence of wound infection.
