牵拉整复法治疗寰枢关节错位所致颈性眩晕的疗效研究

摘　　要：目的:观察牵拉整复法治疗寰枢关节错位所致颈性眩晕患者的临床疗效及颈椎张口位片椎体间的位移变化。方法:70例寰枢关节错位颈椎病患者随机分为两组,治疗组和对照组各35例。两组患者均先行颈部软组织理筋松解后,治疗组以牵拉整复法治疗,对照组则予颈椎牵引治疗,治疗周期为2周,治疗结束后随访1个月,观察手法疗效以及治疗前后患者颈椎张口位片枢椎齿状突位移变化。结果:治疗组治疗后及随访1个月时患者颈性眩晕、颈肩疼痛及生活质量改善情况明显优于对照组,差异有统计学意义(P<0.01)。治疗组患者治疗后、随访时枢椎齿状突偏移程度较治疗前降低(P<0.01),且治疗组疗效优于对照组,与对照组相比有统计学意义(P<0.01)。结论:牵拉整复法治疗寰枢关节错位所致颈性眩晕具有良好疗效,可以纠正寰枢关节骨错缝,且疗效优于颈椎牵引治疗。Objective:To observe the clinical effect of pulling chiropractic method in the treatment of cervical vertigo caused by atlantoaxial joint dislocation and the changes of displacement between vertebral in open mouth view X-ray.Methods:70 cases of atlantoaxial dislocation cervical spondylosis were randomly divided into two groups,35 cases in the treatment group and 35 cases in the control group.The treatment group was treated with pulling chiropractic method,and the control group was treated with cervical traction.The treatment session was 2 weeks.After the treatment,the patients were followed up for 1 month.The curative effect of manipulation and the displacement of the axis odontoid process on the open mouth view X-ray before and after the treatment were observed.Results:the improvement of cervical vertigo,neck pain and quality of life in the treatment group was significantly better than that in the control group after treatment and 1 month of follow-up(P<0.01).After treatment and during follow-up,the deviation degree of axis odontoid process in the treatment group was lower than that before treatment(P<0.01),and the curative effect of the treatment group was better than that of the control group,which was statistically significant compared with the control group(P<0.01).Conclusion:Pulling chiropractic method is effective in the treatment of cervical vertigo caused by atlantoaxial joint dislocation.It can correct atlantoaxial joint bone dislocation,and the effect is better than cervical traction.
