摘　　要：目的:总结一种治疗小儿尿道下裂术后残余阴茎阴囊转位的手术经验。方法:2019年1月—2021年10月通过经隧道阴茎前移法矫治尿道下裂术后残余阴茎阴囊转位患儿18例。沿阴茎根部环形离断阴茎皮肤,游离松解尿道,在阴囊中隔正上方作梭形切口,将阴茎经皮下隧道自梭形切口穿出,缝合固定。观察其临床效果。结果:18例患儿手术时间38~70 min,平均50 min;出血量2~6 mL,平均3 mL;住院时间3~7 d,平均5 d;术后随访2~24个月,平均6个月。所有患儿术后效果理想,外阴外观正常,排尿正常,无切口裂开、尿瘘、尿道狭窄等并发症。结论:经隧道阴茎前移法治疗小儿尿道下裂术后残余阴茎阴囊转位可获得良好的临床效果,外观佳,创伤小,并发症少。Objective:To summarize surgical experience for treatment of pediatric residual penile and scrotum transposition after hypospadias repair surgery.Methods:From January 2019 to October 2021,18 children with residual penile and scrotum transposition after hypospadias repair surgery were treated by penis forward method through tunnel.We detached the penis skin around the base of the penis,released urethra,made a fusiform incision above the scrotal septum,threaded the penis through a fusiform incision through a subcutaneous tunnel.Clinical effects were observed.Results:The operation time of 18 patients was 38-70 min(mean,50 min),and the blood loss was 2-6 mL(mean,3 mL).The hospital stay was 3-7 days(mean,5 days).All the children were followed up for 2-24 months(mean,6 months),and the postoperative results were satisfactory,with normal vulva appearance and normal urination,without incision dehiscence,urinary fistula,urethral stricture or other complications.Conclusion:The treatment of pediatric residual penile and scrotum transposition after hypospadias repair surgery by penis forward method through tunnel can obtain good clinical effect for its good appearance,less trauma and fewer complications.
