超重或肥胖2型糖尿病患者的舌象相关因素研究

摘　　要：目的:探究超重或肥胖2型糖尿病(T2DM)患者的舌象与年龄、体质量指数(BMI)、并发症、实验室指标之间的相关性。方法:应用DS01-G手持式舌象仪采集超重或肥胖T2DM患者的舌象,同时收集其他观测指标。结果:青年全舌质Lab-L值高于老年;青年齿痕出现频率高于年轻老年和老年。全舌质Lab-L、全舌质Lab-b、全舌苔Lab-b与体质量指数(BMI)正相关;肥胖出现苔腻频率高于超重。伴有慢性并发症苔腻出现频率高于不伴有慢性并发症;伴有慢性并发症点刺出现频率低于不伴有慢性并发症;大血管并发症全舌质Lab-a值小于无并发症。舌色参数与空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白B(ApoB)、尿酸有相关性。通过回归分析得到4个回归方程:FBG=64.045-0.409×全舌Lab-b,TC=1.251+0.034×全舌苔Lab-L,LDL-C=-16.986+0.139×全舌苔Lab-a,Apo B=13.111+0.028×全舌苔Lab-a。结论:超重或肥胖T2DM病患者的舌象特征与年龄、BMI、并发症、实验室指标有相关性,并对一些实验室指标有预测作用。Objective:To explore the correlation between tongue manifestation and age,body mass index(BMI),complications,and laboratory indicators in overweight/obese type 2diabetic patients.Methods:The tongue manifestations of overweight/obese type 2diabetic patients were collected using portable tongue diagnosis instrument(DS01-G),and their other observation indicators were also obtained.Results:The Lab-L value of the whole tongue texture in the young group was higher than that of the old group,and the young group also had higher frequency of tooth marks than the young-old group and the old group.The Lab-L and Lab-b values of whole tongue texture and the Lab-b value of whole tongue coating were positively correlated with BMI.In terms of the frequency of greasy coating,the obese group was higher than the overweight group,and the group with chronic complications was higher than the group without chronic complications.Spotted tongue was less frequently observed in the group with chronic complications than the group without chronic complications.The Lab-a value of whole tongue texture in the group with major vascular complications was lower than that in the group without complications.Tongue color parameters were correlated with fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG),triglycerides(TG),total cholesterol(TC),low-density lipoprotein(LDL-C),apolipoprotein B(ApoB),and uric acid.Four equations were obtained by regression analysis:FBG=64.045-0.409×Lab-b of whole tongue,TC=1.251+0.034×Lab-L of whole tongue coating,LDL-C=-16.986+0.139×Lab-a of whole tongue coating,and ApoB=13.111+0.028×Lab-a of whole tongue coating.Conclusion:The tongue manifestations of overweight/obese type 2diabetic patients are correlated with age,BMI,complications,and laboratory indicators,and can predict some laboratory indicators.
