摘　　要：目的探讨髋关节发育不良(DDH)CroweⅢ型患者行THA时在不同位置安放髋臼杯其臼杯的大小和宿主骨覆盖率。方法通过对纳入的2017年9月至2021年10月于哈尔滨医科大学附属第一医院就诊的20例DDH CroweⅢ型患者进行骨盆、股骨三维模型重建,分别于真臼位置、假臼位置、真假臼之间骨量最丰富处选择合适尺寸的髋臼杯进行模拟安放,并计算髋臼杯宿主骨覆盖率。得出数据分为真臼组、中间组、假臼组。结果中间组及假臼组重建的臼杯尺寸大于真臼组,真臼组与其他两组的差异均具有统计学意义,中间组与假臼组臼杯尺寸差异无统计学意义。各组的髋臼杯覆盖率均值都在75%以上,其中中间组与假臼组差异具有统计学意义。结论对于DDH CroweⅢ型患者的THA来说,于假臼或真假臼之间安放髋臼杯可获得较大的臼杯尺寸和满意的宿主骨覆盖率。Objective To explore the cup size and host bone coverage ratio of acetabular cups at different positions where they are placed in THA for DDH Crowe typeⅢpatients.Methods 3D models of pelvis and femur of 20 patients with Crowe typeⅢDDH who were treated in the First Affiliated Hospital of Harbin Medical University from September 2017 to October 2021 were reconstructed.The appropriate cup size was selected for implant simulation at the true acetabulum,the false acetabulum,and the site where the most abundant bone stock between the true and false acetabulum locates,then the acetabular cup host bone coverage ratio was calculated.The data were divided into true acetabulum group,intermediate group,and false acetabulum group.Results The mean size of the reconstructed acetabular cup of the intermediate group and false acetabulum group was larger than the true acetabulum group,and the difference between the true acetabulum group and the other two groups were significant,and there was no significant difference in acetabular cup size between the intermediate group and false acetabulum group.The mean host bone coverage ratio in each group was above 75%,and the value of the intermediate group was significantly different from that of the false acetabulum group.Conclusion In THA for DDH Crowe typeⅢpatients,acetabular cup placement at the false acetabulum or the site between true and false acetabulum can obtain larger acetabular cup size and satisfactory host bone coverage ratio.
