护士用住院精神科患者观察量表及“知情同意”的调查分析

摘　　要：目的:为了进一步尊重精神病患者,调查住院精神病患者的“知情同意”情况,在此基础上,探讨护士用住院病人观察量表(NOSIE)和阳性与阴性症状量表(PANSS)在临床实践中的价值。方法:应用知情同意问卷对山东省淄博市第五人民医院和上海市精神卫生中心患者就入院、检查、服药、夫妻交流等11个问题进行现场调查。在此基础上,对198例精神分裂症患者进行量表评估。结果:共收到有效问卷198份,结果显示87.9%的患者对住院是否必要予以肯定,同意检查者占85.9%,住院不便占52.5%,夫妻交流仅占25.8%。NOSIE和PANSS两个量表显示,本组患者以阴性症状为主要临床特征。结论:精神科患者的“知情同意”执行中还有待于进一步完善。NOSIE护士量表和PANSS量表能全面反映患者生活自理情况及社会功能情况,且易操作,可作为临床护士评价患者的主要工具之一。Objective:To investigate the"informed consent"of hospitalized mental patients to further respect the mental patients.On this basis,the value of nurses'observation scale for inpatient evaluation(NOSIE)and positive and negative syndrome scale(PANSS)in clinical practice were discussed.Methods:The informed consent questionnaire was used to investigate the 11 questions about admission,examination,taking medicine and communication between husband and wife in Shandong Zibo Fifth People's Hospital and Shanghai Mental Health Center.On this basis,198 patients with schizophrenic were assessed with NOSIE and PANSS.Results:198 valid questionnaires were received.It suggested that 87.9%of the patients affirmed the necessity of hospitalization,85.9%agreed to the examination,but 52.5%were inconvenient to hospitalization,and only 25.8%of the couples communicated with each other.NOSIE and PANSS showed that negative symptoms were the main clinical features of the patients in this study.Conclusion:The implementation of"informed consent"of psychiatric patients needs to be further improved.NOSIE and PANSS could fully reflect the selfcare and social function of patients,and can be used as one of the main tools for clinical nurses to evaluate patients.
