归芍地黄汤合二至丸治疗肝肾阴虚型糖尿病肾病的临床研究

摘　　要：目的:分析归芍地黄汤合二至丸治疗肝肾阴虚型糖尿病肾病(DN)的效果。方法:选取2020年2月至2022年2月我院肝肾阴虚型DN患者108例,采用随机分组法分为观察组(n=54)、对照组(n=54)。对照组采用常规西药治疗,观察组在对照组基础上给予归芍地黄汤合二至丸。统计治疗效果,比较两组治疗前后中医证候积分、肾功能[血尿素氮(BUN)、血肌酐(Scr)、尿白蛋白排泄率(UAER)]、氧化应激指标[超氧化物歧化酶(SOD)、沉默信息调节蛋白4(SIRT4)]。结果:观察组总有效率为92.59%(50/54),高于对照组的77.78%(42/54)(P<0.05);治疗后两组中医证候积分、BUN、Scr、UAER水平均低于治疗前,且观察组低于对照组(P<0.05);治疗后两组SOD、SIRT4水平高于治疗前,且观察组高于对照组(P<0.05)。结论:归芍地黄汤合二至丸治疗DN患者效果确切,可有效缓解临床症状,促进肾功能恢复,改善氧化应激状态。Objective:To analyze the effect of Guishao Dihuang decoction and Erzhi pill on diabetic nephropathy(DN)with Yin deficiency of liver and kidney.Methods:108 DN patients with liver and kidney Yin deficiency in our hospital from February 2020 to February 2022 were selected and divided into observation group(n=54)and control group(n=54)by block random grouping method.The control group was treated with conventional western medicine,and the observation group was given Guishao Dihuang decoction and Erzhi pills on the basis of the control group.The TCM syndrome score,renal function[blood urea nitrogen(BUN),creatinine(Scr),urinary albumin excretion rate(UAER)],oxidative stress indicators[superoxide dismutase(SOD),silent information regulatory protein 4(SIRT4)]before and after treatment were compared between the two groups.Results:The total effective rate of the observation group was 92.59%(50/54),which was higher than 77.78%(42/54)of the control group(P<0.05).After treatment,the levels of TCM syndrome score,BUN,Scr and UAER in the two groups were lower than those before treatment,and the observation group was lower than the control group(P<0.05).After treatment,the levels of SOD and SIRT4 in the two groups were higher than those before treatment,and the observation group was higher than that in the control group(P<0.05).Conclusion:Guishao Dihuang Decoction and Erzhi pill have definite effect on DN patients,which can effectively relieve clinical symptoms,promote renal function recovery and improve oxidative stress.
